2.  Enter W, F or C for wedding, funeral or cremation where appropriate, plus the name of the person(s) for whom , the service is conducted

3.  Enter the name of the officiant.

4.  Enter the PDBF's statutory fee receivable for the service, per the extant fees table. Overleaf the table for 2013 is shown. The amount entered should not include the PCC's statutory fee, nor any other non-statutory, optional fees for ancillary services agreed with those for whom the service is conducted. Where the incumbent or priest in charge has exercised his/her statutory right to waive the PDBF's fee, this should be made clear by entering zero, and an explanation given in writing attached to the form.

5.  Enter the amount which has been paid by the PCC to the officiant from the PDBF's statutory fee. This amount should be in accordance with the PDBF's current policy on fees.

6.  Any further relevant or useful information in connection with the service may be entered here.

7.  This form should be sent with a cheque for the total of column 4 less the total of column 5 payable to , the PDBF at First Floor, Peninsular House, Wharf Road, Portsmouth PO2 8HB. Where payment is made , by BACS please email a copy of the form to at the time payment is made.
