SPECIALIST OUTREACH SUPPORT WORKER

JOB PURPOSE

To provide the highest quality outreach service as part of a team to Service Users in accordance with Company policies, procedures and practices

KEY TASK AREAS AND RESPONSIBILITIES

·  To work independently and as part of a team to provide an outreach service to Service Users according to the agreed Person Centred Plans (PCP’s).
·  To support Service Users and evaluate their experience
·  To assist in developing Service Users knowledge and ability in relation to independent living skills
·  To enable Service Users to access employment, education and training opportunities
·  Participate in direct work with Service Users to increase their capacity to deal effectively with day to day living and specific daily tasks.
·  To carry out basic skills assessments and contribute to the development of PCP’s.
·  To monitor and evaluate the support plans and provide written reports
·  Assist Service Users to maximise opportunities in education, employment and training.
·  To contribute to support plan review meetings for Service Users.
·  To work as part of the multidisciplinary team and develop effective relationships with other agencies, and to participate in and develop inter-agency working.
·  Ensure that accurate and appropriate entries are made in Service Users records, diaries and reports as necessary, strictly in accordance with Company and local policies, procedures and practices
·  Safeguard all Service Users and ensure their safety and wellbeing
·  Work positively and effectively with relatives, advocates and others involved with Service Users
General
·  Ensure awareness that the service operates to agreed budgets and contribute to keeping within these budgets as instructed
·  Exercises vigilance in respect of Health and Safety and promptly report all hazards and/or remedying them where appropriate. Undertake all duties in a manner calculated to minimise or avoid unnecessary risks, personally or to others.
·  Operates at all times in accordance with company policies and procedures, with particular reference to Safeguarding, Whistleblowing, Complaints and Representations and Behaviour Policies
·  Participate in training and take responsibility for personal development
·  Participate in team meetings, supervisions and annual reviews in accordance with Company policy and the standards set by the Regulatory Body
·  Work to promote the centre as a valued, professional asset within its community and also to promote a culture that individuals and staff conduct themselves at all times in a manner that reinforces this image
·  Ensure that all actions are in the interests of the Service Users and the Company.
·  To work to and exhibit the POSITIVE values of the Company and maintain standards of behaviour in accordance with Company policies, procedures and practices
·  To carry out any other reasonable and relevant duties as required

Essential / Desirable
Experience
Relevant experience of working in the Community
Experience of work with autistic spectrum disorders and/or challenging behaviour
Experience of giving instructions and guidance to individuals and/or groups on the application of life skills / P

Skills, Knowledge and Aptitudes
Ability to work independently and as part of a team
Effective communication skills, verbal and written
Ability to record information accurately
Good organisational skills
Commitment to working positively with families and others
Flexibility and willingness to work a shift system including weekends

Qualifications and Training
Health and Social Care NVQ Level 2 OR Level 2 Diploma in Health and Social Care (Adults)
Willingness to work towards:
Heath and Social Care NVQ Level 2 OR Level 2 Diploma in Health and Social Care (Adults)
Willingness to work towards further qualifications as required
Undertake relevant Group induction training on commencement / P
