Population Health
As we continue to evaluate Post-Acute Care for our ACO patients, early Medicare data shows an increase in Skilled Nursing Facility spend from 2014 to 2015 for the ACO patient population. This spend went from $5,633,521 in 2014 to $6,060,848 in 2015, an increase of 7.6%. We will continue to track the spend as the data becomes available. We are working with the ACO Finance Committee to better understand the process in order to make an impact going forward. More to come as we uncover results.
Patient 1st Update
1.Admissions
- The Patient Placement team, under the direction of Olivia Vance and Dr. Coats, have determined that our current Direct Admitting policy, as it stands, will stay in place. Data has shown that our patients are getting appropriate/ prompt care via the ED. Very few of those patients sent to the ED result in Admissions.
- We do STRONGLY recommend that if a patient is sent from the office to the ED, Provider to Provider discussion takes place to expedite the care. The ED is currently streamlining this process to better take care of our patients.
- Very few Health systems allow Direct Admits at this time (Much of this has to do with prior authorization and payment).
2.Patients that have been admitted to the hospital since April 5th have likely been signed up for the Meditech patient portal. It is possible for these newly enrolled patients to see their ambulatory test results/reports, within Meditech’ s portal. This is a very small population, and only impacts those that have been inpatient since the portal go live date in April. There is a delay on the release of results, into the portal, to prevent them from seeing these results for 24 hours for labs and 30 hours for diagnostic test reports. Hospital meaningful use requirements dictates the turnaround times in which we must make these accessible within the portal.
Medical Records Committee Update
As a reminder, the Final Diagnosis / Final Clinical Impression is to be documented within 24 hours of the patient’s discharge/departure. If you receive a CDI / Coding Query seeking clarification on a diagnosis or procedure we ask that you respond to the query within 24 hours of receiving it.

Mark your calendar for Thursday, Sept. 1, 2016 for the annual AMG Provider Wine Meet and Greet. This is a fun, networking event to give all Medical Staff the opportunity to meet new providers. The portal is also a way we hope to further communication between providers. We intend to continue to enhance the portal as we strive to improve communication to and with each other.
