Pop Smith Scholarship Application 2015-2016

Students Eligible to Apply Must Have:

  • Have received services from UMFS/Charterhouse School for at least six months. These include:
  • Treatment foster care
  • Adoption
  • The Child and Family Healing Center
  • Charterhouse School
  • Be enrolled at Courage to Succeed for one semester
  • Have received services from UMFS/Charterhouse School within 10 years of the date of application
  • At least a 2.0 cumulative GPA this academic year
  • Filed a 2015-2016 Free Application for Federal Student Aid (FAFSA)

Five Application Parts:

A complete application packet has five parts. Include each part and arrange your packet in this order:

  1. The “Pop Smith Scholarship Checklist”
  2. A complete Pop Smith scholarship application, including the essay
  3. A copy of your 2014-2015college transcript (unofficial), if applicable
  4. A copy of your 2015-2016 financial aid award letter from the school you are attending
  5. Most universities have sent these to you by April 1st. If for some reason you do not have, it please contact the CHS Administrative Services Manager and we can discuss further.
  6. A copy of the FAFSA Student Aid Report (SAR). The SAR can be reprinted by going to (Do not resubmit your FAFSA application.)

How to submit your scholarship application:

  1. Mail it via the post office with the rest of your packet OR
  2. Email it to .

If you choose to email your scholarship application, all deadlines still apply and parts 1, 3, 4 and 5 must be received by the due date, November 30, 2015.

You may also print the application, complete it by hand and mail it to the CHS office. Again, all parts must be received by November 30, 2015 for your application to be considered.

STATEMENT OF VERIFICATION AND RELEASE

I certify that I am eligible for this scholarship and the information provided is true to the best of my knowledge.

If I am selected for the Pop Smith scholarship I will maintain a 2.0 GPA and receive no “F”s. At the end of each semester I will submit my transcript to UMFS personnel to review.
Additionally, if I am selected, I will submit a thank you statement to be shared with Pop Smith donors. As well as attend a Pop Smith Service day event and a reception in celebration of my successes and to thank the donors.

Your permission is needed to request information from the Financial Aid Office of the post-secondary institution the student will be attending. The information will be used to verify the student’s financial aid awards.

Will you authorize the post-secondary institution to release to UMFS all information related to financial aid awards for the student, necessary to evaluate UMFS program services?
