elcome to this issue of our Practice Newsletter. The newsletter is to provide patients with any news, information or forthcoming events. If you have any suggestions as to what you would like to see in your newsletter please inform a member of staff.
Bank Holiday Dates
Please make a note of the dates below of our opening times over the Easter and May bank holidays. If you need a doctor in an emergency please telephone NHS 111.

Repeat Prescription Ordering
The practice no longer accepts repeat prescription requests from pharmacies on behalf of patients. We are aware that the online pharmacy, ‘Pharmacy2U’ is currently mailing Wakefield residents advising of the services that they can provide. Please note that our policy also applies to Pharmacy 2U and therefore if you choose to nominate Pharmacy2U to dispense your medicines, you will still need to order your own prescription from the surgery.
All pharmacies are still able to collect your repeat prescription from the surgery once it has been processed and deliver your medicines to you if they offer these services.
Please be assured that the practice has not provided Pharmacy2U with your details and that we are not affiliated to them in any way.
Staff Training Days
Please make a note of the dates below when the surgery will close.

We will close at 12noon on the day of training and re-open the following day at 8.00am. If you need a Doctor in an emergency please telephone NHS111.
Missed Appointments
During the months of December, January and February a total of 556 patients failed to attend their appointment.
Please contact the surgery if you are unable to attend your appointment then the appointment can be used for someone else who needs it.
Changes of Address and Telephone Numbers
Please let the receptionist know of any changes to your address as soon as possible. Every patient must call into the surgery in person with a completed change of address form obtainable from reception and bring 2 forms of identity, for a list of acceptable forms of identity please ask at reception. If you are unable to get into surgery e.g. you are housebound then please telephone reception for further information. Please let us know of any telephone numbers you can be contacted on, e.g. Home, Mobile or Work as this will provide a more efficient service if we need to contact you. If your home telephone number does not accept incoming calls please provide us with an alternative contact number.
Long Term Conditions-Care Planning
If you have a long term condition it is important you have regularly review appointments with our practice nurses. They will also be able to give you a personal care plan relating to your long term condition. If you haven’t seen the nurse in a while and want to make an appointment please contact the surgery.
NHS Health Check
If you are between the ages of 40 and 74 you are eligible for a free NHS Health Check. An NHS health check is where your risk of heart disease, kidney disease, stroke and diabetes will be assessed through some straight forward tests and simple questions about your lifestyle and family medical history.
If you would like to make an appointment for this, then please contact reception and book with the Practice Nurse and state you would like an NHS Health Check.
Unacceptable Behaviour
We value and care for our staff. We would ask that they are treated with courtesy and respect. Any individual not complying with this request will be asked to leave the premises and may be removed from the Practice List.
