Only leagues or organizations affiliated with US Club Soccer may host U.S. Soccer coaching license courses. A separate application must be completed for each course.
SUBMISSION:The application is due 60 days prior to the proposed course dates. US Club Soccer may approve or reject any application at its sole discretion.

The following checklists outline the requirements and recommendations for hosting a U.S. Soccer “D” coaching license course. Upon review of all submitted materials, US Club Soccer will respond to the hosting organization with approval or denial of the course, and with course fee requirements. Please enter the data below accurately as it will be used to evaluate the host’s ability to run a course.
*Please note that the host organization will be responsible for all course costs exceeding the amount written in 1a., as well as any costs that are not fully covered by the income received from the candidates.

1st Meeting
- Friday Night 5:30-9:30 p.m. (Field from 7:15-9:30 p.m.) Course Overview

HOST RESPONSIBILITIES:The following outlines the requirements and recommendations of the hosting organization’s responsibilities for the course.
The host will be required to do the following:
- Collaborate with the lead instructor to produce a course schedule
- Coordinate with site administration to confirm the facility schedule
- Confirm cooperation/support from on-site administration
- Reconcile all expenses with on-site organization/facility
- Ensure the classroom facility has the following:
- Seats for each candidate with individual writing space
- Projection surface
- Restroom access
- Digital projector linked to classroom-dedicated computer or instructor’s laptop
- Sound system or portable lap top with speakers
- Wi-fi access
- White board / flip chart with markers
- Ensure the field has the following:
- Each candidate will need one-half of a field foreach session (for testing).
- For a ratio of 36 candidates: 3 instructors, course will need at least two full fields.
- Preferred to have 3 full fields (one per instructor).
- Lined and mowed to standard markings
- Restroom access
- Access to water for players & candidates
- Appropriate field equipment
- Portable goals with nets
- 3-4 dozen cones
- Different colored vests
- Vertical poles
- Properly inflated balls
- Medical kit/emergency first aid, including ice/ ice packs for treatment
- Any other field equipment that the instructors find necessary
- Coordinate with US Club Soccer the Proof of insurance for event and participants (including instructors).
