·  Provide care and support to individuals, in line with an agreed support and care plans, supporting people in all aspects of their everyday lives, delivering practical, physical and emotional support.
·  Promote choice, independence and self determination.
What / Key Accountabilities:
·  Provide a high quality service that incorporates the values of the organisation, working within our policies and procedures. Work in partnership with all service users, irrespective of their needs or disability.
·  Provide flexible care and support in accordance with risk assessments and support plans.
·  Support service users in developing and maintaining interesting and enjoyable activities within the community (social, educational, and employment opportunities). Assist service users with planning holidays and accompany them where necessary
·  Undertake household duties, including cooking, making sure the living environment is well maintained, clean and tidy involving service users in carrying out all tasks in accordance with their skills and abilities. Support service users to report and record maintenance issues
·  Provide physical support to service users including support with personal care where necessary
·  Encourage service users to be involved in the practical day to day running of their own home, providing assistance and life skills training that is appropriate to their abilities and which seeks to maximise their level of independence.
·  Respect the confidentiality of the service user and the organisation at all times.

Requirements
Knowledge of risk assessment and support planning is desirable
Good level of English language, literacy and numeracy skills
NVQ level 2 in care is desirable
Experience of providing support to vulnerable people is desirable
Use of IT Skills (Word, Excel Outlook) to prepare and monitor recording information and reports
