We, the undersigning parents of ______,
do hereby consent to our child trying out for the CGHS Varsity Dance Team and participating in the related Dance Team activities at CGHS.
We understand that participating in the CGHS Varsity Dance Team activities involves certain hazards arising from routines and activities. We do hereby agree not to hold the individual coach and/or Center Grove School Corporation responsible should an accident or injury occur as a result of this participation.

CGHS DANCE TEAM FINANCIAL CONSENT FORM
We, the undersigning parents of ______,
do hereby consent to the mentioned financial obligations, and will adhere to the payment deadlines. Please understand that not everything is decided and costs are approximate to what is needed. Accounts must be current each month as CGHS Varsity Dance Team owns all costumes/uniforms and will pull costumes of the student’s whose accounts are more than 30 days past due or over $200 outstanding. Fall team is a six month commitment. Competition teams are a year-long commitment. Students who drop or are removed from the team will not be refunded any fees.

We are sending you this parental consent form to both inform you and to request permission for your child’s photo/image and personally identifiable information to be published on the Center Grove High School Clubs Website and Facebook Page.
Personally identifiable information includes student names, photo or image, locations and times of games. We will not release any of this information without prior written consent from you as parent or guardian.
