PLAN OF SUPERVISION

Introduction and Rationale

Upon receiving invitations to participate in an Out of Town Field Trip, a need arose for the development of a Plan of Supervision. The safety and well being of the students is of utmost regard. The goal of this plan is to ensure proper and constant supervision and safety of the studentsof (School Name)at(Name of Destination). Make sure a plan in place to ensure the safety of ALL students through consistent and appropriate supervision. The plan is outlined below:

Information Collection and Distribution

All students’ and their parents submitted a signedParent or Guardian Approval for School Field Trip form.

Emergency telephone numbers for each student were obtained and will be kept readily available with the teacher(s)/chaperones on the trip and at (School Name) and NE Regional office.

A list of any participating students with special medical problems, along with the names and telephone numbers of the local physicians who treat those special medical problems are filed in the office.

All parents/guardians and all students have been given access to the cell phone numbers of the Teachers, the Administrator, and all of the chaperones that are traveling on the trip.

 All parent chaperones have completed criminal background checks with the District Office.

Supervision during Transportation

Supervision during Activities

Teacher(s)/Chaperones will be positioned around at all times on the front, back, and both sides.

First Aid Kit (or other means of First Aid Treatment) and Emergency numbers will be on hand.

Supervision at Housing Facility  Does Not Apply

Students will occupy reasonable lodging.

All rooms not exceeding quad occupancy. Students per room _____

The rooms will be reasonably segmented off from other rooms occupied by the facility.

There is a “no visitation” policy between studentsand other guest occupying the facility. (if applicable)

All students will have a nightly curfew in which they are required to be in their individually assigned area. Assigned curfew time ______

At curfew, the chaperones will initiate an “area check” to ensure that all students are in their assigned area.

Teacher(s)/Chaperones will stand in the hallways outside the students’ area in shifts such that a chaperone will always be in place.

Miscellaneous Supervision

All of the students will be segmented into groups. Each group will have a Teacher/chaperone as group leader. The students will have to stay with their assigned leader on all excursions, i.e. campus tours, shopping malls, restaurants, museums, amusement parks, etc.

Emergency Situations

ALLincidents that occur on field trips must be reported to the Principal immediately. Serious injuries or fatal accidents please follow the NE Region Reporting and Protocol Procedures (see attachment). Emergencies shall be handled promptly by the Lead Teacher by taking appropriate action. In the discretion and judgment of school personnel or other responsible adult, appropriate action may include sending the student to the hospital or summoning medical aid. In cases where it is necessary to send the student to the hospital, reasonable effort should be made to notify the parents. An accident report is to be filed as soon as possible.

*Make sure you have documented ALL contingencies and reports on file with the Principal and NE Regional Office.

Conclusion

The students are notified of all of the policies governing out-of-town trips. These policies are indicated in the SCS Student Code of Conduct and well as SCS Board Policies. All of the students and their parents are notified that any misbehavior may result in the student being sent home at the parent’s expense.
