Placement Allocation to Portfolio

Practicum A and B

  • The student is required to submit a Request for Placement Form to Charmaine Mahar to provide their contact details and availability for practicum.
  • Upload all DCSI clearances and First Aid Certificate to InPlace:
  • The student will be advised of their allocated agency, the agency supervisor, and the university supervisor.
  • The student must then contact both supervisors to organise an initial meeting to discuss and negotiate hours of attendance at the placement and start and finish dates. The meeting is generally held at the Agency where the prac will be undertaken.
  • At this meeting, the student must:
  • Provide copies of recent Aged Care, Vulnerable People, Disability, and Child clearances
  • Provide a copy of a current First Aid Certificate
  • Complete Part A of the Student Contract.
  • One copy of the Student Placement Contract must be provided to the Agency Supervisor and the University Supervisor.
  • The University Supervisor will forward a copy to the Topic Coordinator
  • The student must include a copy of the contract in their portfolio.
  • The student is required to introduce themselves to other students in their group on the FLO site.
  • The student is also required to post fortnightly progress updates on the FLO site to their university supervisor.
  • The student is required to complete a Timesheet with hours completed. This should be signed by the Agency Supervisor weekly.
  • The University Supervisor will reply to questions on FLO in a timely manner.
  • The student is required to complete Part B of the Student Contract (3 Goals and 3 Key Performance Indicators for each) by week three of their placement.
  • Copies must be provided to the Agency Supervisor and the University Supervisor.
  • The university Supervisor will then forward a copy to the Topic Coordinator.
  • The university supervisor is required to communicate via email with the agency supervisor weekly for the first 4 weeks of the placement to ensure that it is progressing smoothly and that the Student Evaluation Package has been received by the agency.
  • The student is required to organise a mid-placement review with the Agency Supervisor and the University Supervisor before the completion of 40 hours of placement.
  • An agenda for the meeting must be forwarded to both supervisors at least three days before the meeting.
  • The student must chair the meeting, take minutes of the meeting and provide them to both the Agency Supervisor and the University Supervisor within 5 working days of the meeting. Templates for the agenda and minutes can be found in the Practicum Manual.
  • The student must inform the University Supervisor of the final date for the placement.
  • At the completion of the placement, the student, University Supervisor and Agency Supervisor should meet to discuss and sign the Student Evaluation.
  • The University Supervisor will forward the original copy to the Topic Coordinator
  • The student should include a copy of the Student Evaluation in the Practicum Portfolio.
  • The student is required to submit the Portfolio electronically via FLO within 5 working days of the completion of placement hours.
  • The University Supervisor should then contact the Agency Supervisor to thank them for accommodating the student.
