NOTICE REGARDING

IMPACT OF ANTHEM, INC. DATA BREACH

ON CERTAIN PIKE MEDICAL PLAN MEMBERS

On February 25, 2015, Pike was notified by Blue Cross Blue Shield of North Carolina (BCBSNC) that certain current and former members of Pike’s medical plan were affected by the data breach resulting from a recent unauthorized access to Anthem, Inc.’s (Anthem) information system. Anthem is an affiliate of BCBSNC, the third party administrator and network provider for the Pike medical plan. The breach reportedly began in early December 2014 and was discoveredby Anthem on January 29, 2015.

The data breach may have affected Pike members who received services over the last 10 years in any of the 14 states that Anthem services, which include California, Connecticut, Colorado, Georgia, Indiana, Kentucky, Maine, Missouri, Nevada, New Hampshire, New York, Ohio, Virginia and Wisconsin.

Anthem has advised that the information accessed as a result of the breach may include names, birth dates, health care ID numbers, home addresses, email addresses and employment information, including income data. However, Anthem has also advised that no Social Security numbers of Pike members were accessed and that, at this point, there is no evidence that credit card, financial, claims or medical information was compromised.

Anthem is mailing letters directly to all affected members,with more information about the breach and the steps Anthem is taking to mitigate harm to affected individuals. Anthem is offering credit monitoring, identity theft protection and credit repair services at no cost to affected individuals. If you believe that you may have been impacted by the breach and you do not receive the letter directly from Anthem in the next fewweeks, you may call the Pike Benefits Department at 866-621-2504and we can providea copy.
