Physiotherapist (PT) – Cardiac Rehabilitation Exercise Program

Responsibilities:As a member of the inter-professional team, thePhysiotherapist is responsible for the provision of primary health care services in accordance with the Standards of Practice from the College ofPhysiotherapistsof Ontario. The PT will primarily be responsible for the coordination, implementation, monitoring and evaluation of the Cardiac Rehabilitation Exercise Program for clients referred to SVCHC through the University Of Ottawa Heart Institute (UOHI), and other local hospitals. The Physiotherapist will be responsible to assess participants' exercise capacity and program eligibility as well as provide and implement exercise prescriptions based on results of patient's exercise stress test or metabolic tests considering clinical status and patients' response to exercise. Physiotherapist will provide education to patients and their families and other health care professionals.Cardiac Rehabilitation Exercise Program is delivered within a disease prevention and health promotion framework using telemedicine.

Qualifications: Bachelor's degree in Physiotherapy and current practicing membership with the College of Physiotherapists of Ontario. Masters degree in exercise physiology and/or certification with the American College Sports Medicine (ACSM) as an exercise specialist or with the Canadian Society for Exercise Physiology (CSEP) as a Certified Exercise Physiologist would be preferred. Three to five years of recent related experience in a chronic disease rehabilitation environment, including evaluating and interpreting clinical data, conducting group exercise sessions, responding to complication, modifying exercise for cardiopulmonary limitations, prescribing aerobic and strength exercise, individual counseling, and care plan development. A demonstrated ability to work effectively in aninter-professional team environment and ability to work with minimal supervision in a high stress environment. Sensitivity to the needs of the disadvantaged and experience with a diverse population. Well developed inter-personal and communication skills are required. Proficiency working in a computerized environment and experience working with electronic client records is required. Bilingualism is preferred.
