PHYSICIAN RECRUITMENT INCENTIVE PROGRAM

PURPOSE

The purpose of the physician recruitment incentive program is to encourage employees and volunteers of Sample Health and its affiliates to solicit and identify interested and qualified physician candidates in specialties identified in our Medical Staff Development Plan or otherwise targeted.

ELIGIBILITY

Active full-time, part-time and casual employees and all volunteers of Sample Health affiliates. Physicians are excluded from participation. Also excluded from participation are administrators and those for whom physician recruitment is an essential part of their job responsibility including, but not limited to, Strategic Business Unite leadership, physician recruiters, and departmental or practice managers.

PROCEDURE

Any employee or volunteer with knowledge of a physician, in the identified specialties we are actively recruiting, should forward that information to the Physician Recruitment Coordinators.

If that candidate has not been previously identified or evaluated by our physician recruitment coordinators, and if subsequent CV evaluation, telephone interview, or other evaluation methods result in an on-site visit to a Sample Medical Center by the physician candidate under the coordination of our physician recruiters, the employee or volunteer is eligible for a $500 incentive award. The site visit award will be given only once per candidate (there may be multiple site visits by a candidate). The award will be processed immediately following the completed site visit.

Should that same candidate subsequently join the medical staff of one of our four medical centers within 18 months of the completed site visit, the employee or volunteer will be eligible for an additional $500 incentive award. The employee must be actively employed at that time in order to receive the additional award. The award will be processed after the first day of practice by the new physician.

PAYMENT PROCESS

Upon successful completion of the site visit or start of the physician practice as a member of our medical staff, the award will be submitted for payment. The payment amount to the employee or volunteer will be $500.00 upon completion of a site visit and $500.00 when the candidate begins work as an active member of our medical staff.

For an employee receiving the award, the amount will be processed with the normal payroll system; appropriate taxes will be deducted from this gross amount.

For a volunteer receiving the award, a check will be processed as miscellaneous income. The volunteer will be solely responsible for any tax reporting requirements and liability that may arise due to receipt of this award.

Final determination of the eligibility for the award and or resolution of any disputes arising out of this process will be made by the Physician Recruitment Coordinators and the Vice President responsible for physician recruitment, in consultation with the Vice President for Human Resources.
