General Guidelines:
1. Nomination forms can be submitted by anyone who feels they know a worthy nominee.
2. Current employees of St. John Vianney High School are not eligible for membership.
3. Nominations Forms and supporting data must be received by Pete Cerone () no later than January 31, 2018.
4. All four criteria are weighted. A nomination must address all four areas in detail.
5. Each of the four areas will be rated from a 1 (no data to support) to 10 (excels) by the Selection Committee. They will use this nomination as a guideline for their ratings. The person making this nomination does not rate each category. His/her responsibility is to provide as much information as possible in each category so a proper rating can be made by the Selection Committee.
The quality and quantity of data for each area will help the nominee in the selection process. Please articulate all supporting data for each of the four areas for your nominee. The explanations should be attached to the Nomination Form and submitted to Mr. Pete Cerone via email by January 31, 2018.
