Inspections are to be completed weekly on all manned facilities and during routine visits to unmanned facilities. Personnel working a 14 X 14 schedule shall complete 2 (two) checklists per hitch. This checklist should be completed within the first 24 hours of the hitch. Any deficiencies shall be reported to the Fieldwood Energy Production Foreman immediately.

1 / Are PSV isolation valves locked in the open position?
2 / Are LSHL isolation valves in the open position?
3 / Are LSHL isolation valves tie-wrapped in the open position?
4 / Have the LSHL devices on the sump/blowcase, flare scrubber and oil storage tanks been function tested? (To be done weekly)
5 / Are all safety system panels in the “in service” position?
6 / Is the SCSSV isolation needle valve in the “open” position on all flowing wells?
7 / Are the SCSSV closed and the control line disconnected on all shut-in wells?
8 / Are gas detection panels in normal operating status?
9 / Is there any grating or handrails missing or damaged?
10 / Are all swing ropes in good condition?
11 / Are the fog horn and Nav-Aid lights in good working order?
12 / Is the general alarm system operating correctly?
13 / Are there any exposed wires or broken junction boxes?
14 / Do all qualified personnel have Fieldwood Energy standard red Bypass tags?
15 / Is there a portable gas detector and calibration gas available and in good working condition at this facility?
16 / Has all safety and life saving equipment been checked for proper working condition?
17 / Has diesel fire pump engine and auxiliary generator been started?
18 / Are good housekeeping conditions being maintained at this facility and all loose gear marked with the platform location?
19 / Are all chemical containers and other tanks labeled with contents and NFPA diamond information (MSDS provided)?
20 / Is all out of service equipment and/or electrical panels labeled and locked and tagged?
21 / Has crane boom been inspected for damages? If damage is noted IMMEDIATELY NOTIFY YOUR SUPERVISOR

Note: Fieldwood Energy issued standard red bypass tags are to be used when testing all safety devises.
