COORDINATED SUPPORT PLAN (CSP)
Instructions: Program Specialist (PS) shall review the Data Sources and summarize the findings in the corresponding Comprehensive Data and Needs Assessment Review section. Based on this review, the PS shall work with school staff to determine how the PS can best support the school by developing Action Items (along with Person(s) Responsible, Timeline, and Progress Monitoring) that are aligned with the required levels of support.For additional information refer to theCoordinated Support Plan Information Handout.
Consultation and Monitoring / Coordination and Facilitation / Comprehensive Coaching
PS provides limited support mainly in a consultative manner. Monitoring is provided via desktop and may include additional monitoring activities as determined necessary by the review. Example: Review EasyIEP for compliance and following up with e-mail. / PS will coordinate training and facilitate meetings to address action items, identified during the data review, to support the action item(s). PS will also continue to monitoractivities as determined necessary by the review. Example: Coordinate consultation with autism Itinerant for classroom support or to address a specific student need. / PS will coordinate training and facilitate meetings, as necessary, to support the action item. PS will also provide focusedprofessional development, guidance and continuous monitoring. Example: Utilize a team approach to provide intensive support by involving additional program specialists with various areas of expertise to address school needs
