Purpose

Create a two-way discussion about present and recent past performance and provide a common understanding for future growth.

Process (action steps)

Follow these steps to complete your performance assessment:

1.  Complete the Motivators section below, the self-assessment sections for each of the seven (7) roles (see right), and the self-assessment section for the Key Metrics and Summary. Save form to Desktop or My Documents.

2.  Attach and e-mail this form to your District Manager (DM)/JVP. He or she will complete the DM/JVP sections for each role, the key metrics, the summary, and the overall performance rating.

3.  Meet with your DM/JVP to compare and discuss performance. He or she should bring supporting documents (cafe results, audits, etc.) to the meeting. After discussing performance, your DM/JVP will review The Motivators section with you to optimize your Panera experience.

4.  Your DM/JVP will provide a copy of the completed form to you and place one in your personnel file.

5.  Finally, your DM/JVP will meet with you to discuss your Individual Development and/or Action Plan.

Section Two – The Roles
