1. Instructions to Bidders
a. The Department of Environmental Protection (DEP), Bureau of Office Services, is requesting price bids for the following items listed in the Invitation for Bid (IFB) item list section. The bidder must supply a price for all items. Award will be made on the basis of the lowest grand total price.
2. Delivery
a. Physical delivery location: DEP ATTN: Debbie Perkins, Clerical Support Section District Mining – Pottsville Office, 5 West Laurel Blvd., 2sn Floor, Pottsville, PA 17901.
b. Please include all costs including shipping/handling within the costs of the line items supplied, i.e. bid prices must be FOB destination. No additional costs will be allowed.

4. Estimated Quantities
a. The quantities contained in the IFB section are estimated only and may increase or decrease dependent upon the needs of DEP. Items may be added to this acquisition at the discretion of the DEP at the price originally bid.
b. The contractor shall be paid at the unit bid price for the quantities actually supplied.
5. Requirements
a. See (IFB) item list section for listing of parts required.
b. Laval Underground Surveys (Or approved equal) is the requested manufacture for all items contained in this bid.
c. If you wish to substitute with an approved equal please contact to Debbie Perkins, phone 570-621-3118, email at least five working days prior to IFB opening date.
d. The Department (DEP) shall issue final approval in writing whether the substitute will be accepted.
6. Award:
a. Award will be made to the responsible responsive bidder on the basis of the lowest grand total price.
b. Bidders are required to complete and return the “Reciprocal Limitations Form” electronically attached to the IFB file.
