All dogs & cats 6 months or older are required to be licensed by

January 31 of every year!

If you wish to apply through the mail please fill out the information above and send with:

1.  Self addressed stamped envelope

2.  Copy of current rabies certificate

3.  Copy of proof of Spay/Neutering (if applicable)

4.  Check or money order made payable to:

50 Egbert Street Neutered/Spayed $12.00

Pemberton, NJ 08068 Non-neutered $15.00

Late fee $5.00 per month additional

License applications can also be brought to the Municipal Building Monday through Thursday 7:30am to 4:30pm.

If you no longer have your pet please inform this office so we may keep our records up to date.

Per State law the rabies vaccination must be good until at least November 1, 2017 in order for us to issue a registration.

This application may be copied for use of more than one pet.
