Welcome to Dr Ahmed's Practice

We wish to thank you for registering with our Practice. The Partners and staff of this long-established practice are committed to providing the highest level of patient care.

You have the flexibility of consulting with either your registered doctor or one of the other GPs We offer a choice of mixed age, male or female doctors and therefore hope that we will be able to match your needs for a specific consulting style.

However, we recommend you see the same doctor for any ongoing problem to ensure continuity of care.

This booklet contains information about the facilities and services at the Practice and we hope you will find it useful and keep it handy for reference.

THE DOCTORS:

Patients are free to consult whichever doctor they choose, irrespective of the doctor with whom they are registered. However, it is best if some continuity can be maintained by seeing the same doctor for each problem.

If you have a preference for a particular doctor, please inform our Reception Staff.

All GPs are registered with the General Medical Council.

Shamim Ahmed is the manager for the Practice and she may be able to help you with any administrative or non-medical aspects of your health and treatment.

She is also available to discuss any suggestions or complaints. A suggestion box is available in the waiting room.

Our Reception Staff are here to help you. When telephoning for medical attention our Reception Staff may ask for a few details. The doctors have asked them to make these enquiries so that they can help you in the most appropriate way. Our Reception Staff have undertaken special training and always respect patient confidentiality.

Practice Nurse(s)

Nur Begum and Sohail John are the Practice nurses and they are available by appointment for various treatments, health promotion advice and screening, including smears, immunisations and vaccinations except Fridays.

The nurses are qualified to advise and treat many minor conditions and it may prove quicker to see a Practice nurse than wait for a consultation with your doctor, who is always available if you have any cause for concern.

When booking an appointment with a Nurse Practitioner or a Practice Nurse, our Reception Staff may need to ask you the reason for an appointment in order to determine how much time to allocate. Consultations are by appointment.

Nursing Assistant(s)

Regina Rodrigues is our Health Care Assistant

She is not qualified as a nurse, but has undergone training to enable her to take bloods, blood pressure, assist with minor surgery, new patient health checks, flu vaccine, obesity monitoring that do not involve medication.

She assists Dr. Ahmed with our 24-hour Blood Pressure Monitoring Clinics when needed.

All requests for blood tests must be made through one of the doctors, nurse consultant(s) or nurse practitioner(s).

ATTACHED STAFF

Midwife

Our attached midwife is Angela Callender

Midwives care for and support pregnant women, their partners and new babies before, during and after the birth. They monitor the health of the mother, counsel her on health issues and explain the options for delivery of the baby.

Their job also involves reassuring parents, running antenatal and parenting classes, taking care of the mother and baby during labour and birth and giving advice on breast feeding.

District Nurses

District Nurse: Patricia Brown

District nurses assess, plan and manage the care of sick and disable patients of all ages in the patient's own home, general practices and residential nursing homes and also provide support for their carer.

Individual care is planned, and advice on further help or services is given including health education. Anyone at home can self-refer to this service. They have a close liaison with the Hospital, GPs, the local hospice and other specialized services.

They administer drugs, give injections, dress wounds, take blood samples and give personal care.

The district nurses are in regular contact with the Practice and messages may be left on :

Health Visitors

Bola Oladele attached to the Practice.

They operate out of Ballam Street Clinic and are usually available by telephone

Her telephone number is 020 8218 7414

Health visitors are qualified nurses and have additional training in child health and development, will monitor your child’s growth and development, answer any child health queries and help mothers to cope with children under five, advising on things like hygiene, safety, feeding and sleeping. They also co-ordinate child immunisation programmes and organise special clinics or drop-in centres.

Health visitors can also provide you with help and information regarding such issues as emotional problems, relationship difficulties, family planning and health problems and advise on healthy eating, keeping warm and getting the right exercise.

Emergencies

For life-threatening emergencies such as:

·  Severe bleeding;

·  Collapse or unconsciousness;

·  Severe chest pains…

Telephone 999 for an ambulance IMMEDIATELY, then inform your doctor.

When the Surgery is closed

We also close between 12:30 to 4Pm for training purposes on Thursday each month. The dates are posted in the waiting room and on the front door.

For emergencies that cannot wait until the next surgery, please telephone 020 3668 0788, when you will be advised how to contact the duty doctor.

Please remember that this service is there to provide urgent medical attention for ill patients. If the service is abused by calling the doctor for requests for repeat prescriptions, appointments or minor illness which could wait until the next surgery session, then other patients could be put at risk.

When the Practice is closed your call will be diverted to the Out-of-Hours service. You may also telephone the Out-of-Hours service directly on 020 7511 8880.

If you are unable to contact the doctor and the emergency persists, ring 999 without delay.

Making an Appointment

All our regular weekday surgeries are by appointment only.

Please see our Reception Staff or telephone the Practice during Opening Times on 020 3668 0788 to arrange an appointment to see the doctor or nurse on the day.

Regular routine appointments may be made well in advance up to a maximum of 4 weeks

Clinic appointments (such as Asthma or Diabetes) can be booked up to a maximum of 2 weeks in advance.

The doctor will try their best to give enough time to each patient but if, for example, two members of one family try to be seen in a single appointment, other patients will be kept waiting. So please make an appointment for each person wishing to be seen.

If you think that your problem may take an unusually long time, please inform our Reception Staff so that allowances can be made for this.

If your condition is non-urgent, you can expect to see a doctor within 3 weeks though you may have to wait longer if you want to see a particular doctor.

You can expect to see a nurse within 10days or less.

Urgent Appointments

If you have an urgent medical problem that you feel needs attention on the same day, please explain this to our Reception Staff who will invoke our telephone triage service to deal with such urgent requests.

When you call, our Reception Staff will take your name and telephone number and either the duty doctor or nurse practitioner will telephone you back as soon as possible (usually within 2hrs hour).

If the clinician thinks you need to be seen, you will usually be offered an appointment that day. Please note that this service is not available during the additional appointment times specified above.

Cancelling Appointments

If you are unable to keep your appointment, please let us know as soon as possible so that we can allocate it to someone else.

We have a high DNA ( Daily Non Attendant) rate. Therefore patients are requested to cancel their appointment if they no longer need it so that another patient can be seen.

You can also cancel your appointment outside of normal working hours by going on- line. Please ask at the reception.

Access to Services by people with Disabilities

The Practice has done its utmost to be easily accessible and user-friendly for our disabled and wheelchair-bound patients.

There are designated parking spaces in the car park.

A wheelchair is available for those with difficulty in walking - this can be obtained through our Reception Staff.

An induction loop is available to help the hard of hearing have clearer conversations. This can also be provided for individual consulting rooms when necessary.

There is a ramp at the surgery entrance and also inside to aid wheelchair access.

Surgery rooms located on the first floor are accessible by using the lift.

There are also toilet facilities for the disabled at the ground level of the premises.

If you experience any problems, please speak to our Reception Staff, who will do their utmost to assist you.

Other Access Issues

Prams and Bicycles

Prams and bicycles may be left in the outer foyer of the main entrance to the Practice at your own risk. A metal wall bar is available for securing bicycles, however patients should provide their own chains / locks.

Dogs

All dogs, with the exception of assistance dogs, should be left outside the building. There are metal rail available to secure a leash if required outside at the front of the building.

Smoking & Food

The building is a non-smoking building and we request that this be honoured.

We would also request that patients do not eat or drink whilst waiting for their appointments. A cup of water may be obtained from reception on request.

Self Check-in

The surgery has an automated self -check-in touch screen at the corridor.

This is a simple to use system that enables the patient to check themselves into our appointments system.

Should you feel uncomfortable about using such a system, you can obtain help, or book in with our Reception Staff in the traditional manner.

Home Visits

As home visits are for those patients too ill to attend surgery, please only request a home visit if you (or the person you are calling on behalf of) is too ill or frail to attend the Practice. Please remember the doctor can see four patients in the time it takes to make one house call.

The doctor may arrange for the patient to be brought to the surgery so that its full facilities are available for tests and treatment.

It is not possible to request a specific doctor for a home visit.

Please give clear directions so the doctor can locate your house. If you have an outside light, please ensure that it is switched on during the hours of darkness.

Also, if you have a dog that is not familiar with visitors, please ensure that it is securely locked away.

Out-of-Hours Appointments

Practice patients who have urgent medical problems that will not wait until normal working hours may either ring the Practice on 020 7511 8880 between the hours of 6:30pm to 8:00am and all day during the weekend.

Your call will automatically be transferred to our out-of-hours service who will take your personal details and details of your medical problem.

Alternatively, you may contact the out-of-hours service directly on 020 7511 8880.

The out-of-hours service will contact the most appropriate healthcare professional who will call you back to discuss your problem and, after taking a full history, will either:
