Below is the demonstration account used for this How-To guide. You may use your assigned agency account information to access the database. If your agency does not have an account and would like to request one, please contact Jerry Endres, Project Director, at .

  1. Click on Reports, located on the top left hand side of the screen.

  1. Click on Add New Reportfor the type of report you wish to generate (both reports use the same reporting criteria).

Step One: Select the Specific Subjects for Your Report

Select Specific Clients

There are three pre-selected groups of clients you can select for the report: Select All Clients, Select Active Clients. and Select Inactive Clients.

Select All Clients

By selecting all clients you include active and inactive clients. This means you would also include those clients referred to as “closed” and “deactivated.”

Select Active Clients

By selecting active clients you will not include inactive, closed, and deactivated clients. Active clients are identified in black font inside the scroll box.

Select Inactive Clients

By selecting inactive clients you will include closed and deactivated clients. You will not include active .clients. Inactive clients are identified in red font inside the scroll box.

Unselect Clients

This button is needed to clear the previous selection made.

Select Clients Manually

Select your clients manually through the scroll box. You may click checkbox next to the client code you wish to include in the report.

Select Clients by Grouping Codes Within Subject ID

The first sixcode boxes are mandatory for all agencies and descriptions for the ID codes are available next to each digit or number.

For code boxes 7-32, you will only see numbers and/or letters. For descriptions of each ID code you will need to refer to your agency’s code protocol.

You may select as many ID codes as possible for each code box. By selecting an ID code under a code box you are limiting the reporting criteria.

Codes boxes that are not used to select specific ID codes will not be affected. The system will include all ID codes for code boxes not used.

Example Translation

The system is asked to generate a report that includes all clients in the database so long as they are:Latino-Hispanic, Male head of household, and have exactly 2 children between the ages of 6 and 12 years.

Step two: Refine your search

On step 2, you are able to identify a range in dates and/or assessments (visits) for your report. Below are general descriptions of possible selections:

Assessment Ranges

“ALL to ALL” assessments
When you select ALL to ALL assessments, you are telling the system to include ALL clients who haveany number of assessments (1,2,3,4) completed. This is all inclusive and is helpful to see how many cases the agency has seen.

“1 to 2” Assessments ( and other ranges)

When you select visit "1 to 2", you tell the system to include ONLY clients who have had ATLEAST visit1 & 2 completed. If a client has assessments 1-4 completed, they would be included.

Date Ranges

If you run a report with a specific date range, ONLY assessment numbers within the given timeframe will be included and if there are pre-selected assessment number ranges(1 to 2) fall within the timeframe will be include.

Example of Assessment and Date Range

Report Criteria 1

Clients: All active clients

Assessments from Number: 1 to 2

Assessments from/to Date: January 2011 to December 2011

A client must have both assessment 1 AND 2 completed between January and December 2011. If "Client A" has 4 assessments completed, and had only visit 2 completed between January 2011 and December 2011, sheWOULD NOTbe included in this report.

Report Criteria 2

Clients: All active clients

Assessments from Number: ALL to ALL

Assessments from/to Date: January 2011 to December 2011

A client who has any assessment number completed between January and December 2011 will be included. If "Client A" has 4 assessments completed and had only visit 3 and 4 completed between January 2011 and December 2011, sheWOULD be included in this report.

Step three: Choose Indicators to Target and Finish

You are required to select at least one indicator for each report. You may select indicators manually by check marking the box next to the indicator you wish to add. You may also click on the “Select All” button to include all indicators.

When you are finished selecting your indicators and have confirmed your reporting criteria, click on the Generate Report button.
