St. Columba's Hospice is a busy specialist palliative care unit with over 450 inpatient admissions annually and busy community services with an expanding outpatient service. We have an exceptional clinical environment and are committed to delivering the highest quality care to patients and support to their families. We have an exciting 5 year strategy which looks to develop all services, including our research capacity.

Consultant in Palliative Medicine

Part-time 5PAs (20 hours) per week + on call commitment

£77,529 - £104,525 per annum pro-rata + on-call availability allowance

An exciting opportunity has arisen for a part-time permanent consultant post which will combine clinical and research commitments. You will join our multidisciplinary team as it moves into year 2 of our 5 year strategy which aims to deliver excellent palliative care within Lothian.

You will work with the Medical Director and the other part-time consultant in providing senior medical input to patients with specialist palliative care needs in our busy in-patient unit and community services. You will also work with the Director of Education and Research to develop medical research capacity within the Hospice. Applicants must be fully registered with the GMC and hold a License to Practice and be on the Specialist Register. The Hospice participates in the training of specialty registrars and GPST doctors. Along with colleagues across Lothian, you will also participate in the consultant on-call rota.

Higher degree (MD/PhD) with significant research experience
Full GMC registration and licence to practise.

Section 2. Introduction to Appointment

Job Title: Consultant in Palliative Medicine

Base: St Columba’s Hospice, 15 Boswall Road, Edinburgh, EH5 3RW

Post Summary:

The post holder will play a lead medical role in the delivery of care to inpatients and community patients in collaboration with the Medical Director, other consultant and medical colleagues, the wider multidisciplinary team and the hospice management team.

They will provide a visible, accessible and authoritative presence in the hospice to whom staff, patients and their families can turn to for assistance, advice and support.

The role also involves a commitment to developing medical research activity at the Hospice, working with the Director of Education and Research. This will also involve the development of, and supervision of, a part-time medical research fellow post.

The role also involves participation in the training and educational work of the hospice including the undergraduate medical teaching programme, and clinical supervision of junior medical staff and involvement in service development as part of the Hospice strategy

It is anticipated that the post holder will be granted Honorary status as a Consultant to NHS Lothian, NHS Borders (for telephone advice) and Marie Curie Hospice (Edinburgh).

The appointee will have secretarial support, IT facilities and office accommodation within the Hospice.

Section 3. About St. Columba’s Hospice

The Hospice provides Specialist Palliative Care services to patients and their families and carers across Edinburgh & the Lothians.

St. Columba's Hospice, a Specialist Palliative Care Unit, opened in 1977. It is an independent charity providing its Services to NHS Lothian and also accepts patients from other Health Boards according to need. Currently around a quarter of its revenue requirements come from NHS Lothian, the balance coming from voluntary support.

It is committed to the care of patients with active, progressive, far-advanced disease and to the support of their families.

It has 30 beds in a purpose-built inpatient unit and also provides a range of Community based services, within the City of Edinburgh, including CNS reviews, Day Services and outpatient reviews. It also provides palliative medical input to East Lothian.

There is a comprehensive education and study programme for students and practitioners of the medical, nursing, paramedical, theological and allied professions. This includes a Graduate Certificate in Palliative Care, and a multidisciplinary Masters programme in conjunction with Queen Margaret University, including a Postgraduate certificate, diploma or MSc. Many members of staff are involved in education; the department is overseen by the Director of Education and Research. The Hospice is also expanding its research interests as part of its 5 year strategy.

Section 4. Main Duties and Responsibilities

This job description is not intended as an indication of the full and detailed range of duties that may be undertaken by the post holder and will be reviewed and developed to reflect the changing needs of the service. Other duties required by the Medical Director that are commensurate to the grade of the post may be requested from time to time. Responsibilities will be reviewed regularly with the post holder.

General

The post holder will assist the Medical Director and the other part-time consultant to co-ordinate, support and develop the delivery of specialist palliative medical care in inpatient and community based settings.

Clinical

The post holder will:

·  Act as a key senior member of the Hospice Team, liaising with the Medical Director, other consultant, senior nursing staff and other staff members, ensuring a multi-disciplinary approach to palliative care, and sharing clinical responsibility for the Hospice in-patients with the Medical Director and other consultant.

·  Offer consultant input to the inpatient unit and be a clinical resource for the junior doctors and other staff in the Hospice.

·  Clinically supervise the doctors in training and specialty doctors, providing them with expertise and leadership around clinical decision making.

·  Provide medical support to community services, including clinical nurse specialists, day services and help in the further development of outpatient services.

·  Help provide medical advice and support to the Macmillan Nurses, primary and secondary care in providing palliative care to patients in East Lothian in absence of the Medical Director.

·  Second on-call responsibilities will include giving specialist advice 24/7 to healthcare professionals in Lothian and Borders. Occasionally this may require a visit to one of the Edinburgh hospices or the acute hospitals.

·  Work with healthcare professionals within primary care and acute settings and in the wider community, with other providers of palliative care (NHS, voluntary sector and social services) to co-ordinate and optimise the provision of palliative care.

·  Promote close working relationships with primary care teams, providing clinical advice, out-patient assessments and occasional domiciliary assessments as appropriate.

·  Ensure effective working relationships are fostered, based on good communication between professionals in the shared-care management of patients requiring palliative care.

·  Promote good communication, information and integration of services for patients and their families.

Managerial

The post holder will:

·  Deputise for the Medical Director as appropriate, in his absence, at the Management Committee and other appropriate management meetings.

·  Take personal responsibility for risk management in the area of specialist palliative care and undertake to review practices and learn from mistakes.

·  To participate in relevant Clinical Governance and strategy meetings.

·  Play a role in the development of specialist palliative care services at the Hospice

·  To contribute to the Hospice’s Education programme

·  To observe the Hospice’s agreed policies and procedures

·  To ensure effective working relationships with those agencies concerned with the provision of health and social care services and those representing the interests of patients.

·  To represent St. Columba’s Hospice at external meetings if required.

·  Ensure close collaboration with other Specialist Palliative Care providers across Lothian to ensure a proactive integrated model of palliative care service delivery to all patients within Lothian

·  To ensure compliance with the General Medical Council’s published guide “Good Medical Practice” including the Duties of a Doctor.

·  To have regard for the health and safety regulations in place.

Clinical Governance

The new appointee will be expected to apply the principles of clinical governance:

·  Incorporating clinical effectiveness, risk management and audit into everyday practice and service developments.

·  Promoting evidence based practice and use of Clinical Guidelines.

·  In conjunction with the Medical Director and other consultant, ensure that clinical supervision and support is provided to the palliative care medical team. Recognise informal and formal staff support systems and contribute to staff support when appropriate, whilst accessing personal support when necessary.

·  To contribute to the requirements of Clinical Governance within the Hospice.

·  To participate in, and encourage, clinical and other service audits and developments with the aim of ensuring a high standard of patient care.

Education and Training

·  To play a key role, jointly with other colleagues, and with the collaboration of the education department in the delivery of teaching in palliative care to healthcare professionals and medical students served by the Hospice.

·  A proactive interest in lifelong learning is expected along with engagement in continuing professional development. CPD status must be maintained and the Hospice will ensure adequate funding provision for this.

Research, and Development:

·  Help in implementing the Hospice research strategy by:

o  Developing and supervising the medical research fellow post.

o  Developing research proposals by working with Hospice staff and local/national research networks

o  Working closely with the Director of Education and Research

Professional

·  To maintain evidence of registration with the GMC, be on the specialist register and hold a license to practice.

·  To act in accordance with standards laid down by the GMC, and Hospice policies and procedures.

·  To participate in clinical supervision if appropriate.

·  To participate in the appraisal and revalidation process. To attend education courses and study days to be aware of developments in specialist palliative care and the wider clinical arena.
