Parents of new students to North High School,

If you have not registered your student we will enrolling on Wednesdays and Thursdays from 8AM to noon beginning June 20th and ending July 13th. Please be aware that North High is a separate school district from your student’s Jr High or Middle School, and registration is required of each student prior to school starting in August.

The following documents are required for registration: Enrollment Packet, POR, Picture ID of Legal Guardian, Birth Certificate, Shot Record and Court Documents. If you do not have an enrollment packet you can stop by the North High Office anytime, M-F from 8AM to noon or 1:30 – 3:00 to pick one up.

In addition to the completed enrollment packet you will need to bring with you:

1.  Current Proof of Residence. Acceptable documents are one of the following: (PGE, Gas, Water, Cable or Landline Phone Bill). We also will accept a current Passport of Services in Legal Guardians name.

If the legal guardian is in a shared residence and does not have a major bill in their name then the following documentation will be needed for enrollment:

1.  Current bill or Passport of Services in the Primary resident’s name.

2.  ID of Primary Resident

3.  Signed Shared Residence Form; which can be obtained at North High

The legal guardian will then need to provide:

1.  Two pieces of mail showing they are living and receiving mail at that address. Acceptable documents are:

a.  Driver’s License with correct address – also needed for identification

b.  Bank Statement

c.  Car Registration or Car Insurance

d.  Current Pay Stub

e.  Cell Phone Bill

f.  Passport of Services

g.  Signed Shared Residence Form.

Birth Certificate

Shot Record – most have been sent over from feeder school, but not all. Students, by law, cannot attend school without proof of immunizations being current and up to date.

Court Appointed Guardian or Custody Papers.

If there has been a marriage and divorce or if a student lives with someone other than their biological parents, court documents with a stamp and Judges Signature will need to be provided showing custody.
