Dear SSD Coordinator,
Thanks to special ODE funding, all of your eligible 10th graders can take the PSAT for free at your high school during the regular school day on Oct. 14, 2015. Our Services for Students with Disabilities (SSD) office is available to assist your students who qualify to receive testing accommodations. Please read this flyer for a quick overview of our SSD Online system, designed to help schools request and manage accommodations for students with disabilities. The SSD review process takes time, so please submit your students' requests as soon as you can. The deadline for submitting SSD accommodations requests for the PSAT is August 25, 2015. The College Board cannot guarantee that requests submitted after Aug. 25 will be reviewed and materials shipped in time for the test.
  • Test coordinators can order non standard tests, such as large print, online. To take the PSAT/NMSQT tests with testing accommodations, students with disabilities must have their accommodations approved by the College Board.
  • For students who have not yet applied for SSD accommodations, please submit requests as soon as possible. Status can be checked on the online portal.

Requesting SSD Accommodations
Most requests for accommodations can be submitted using the College Board's SSD Online service. Here are the steps:
  1. Download these forms, if you have not previously provided them to the College Board:
  2. SSD Coordinator Form (.pdf/332K)
  3. Parent Consent Form (.pdf/204K) for each student
  1. Log in to your existing professional account (or create one) at :
  2. Select Disabilities Accommodation Management System.
  3. Click Submit Eligibility Form in top right corner of the dashboard.
  4. Complete the questions and submit the form electronically.
  1. If documentation is required:
  2. Print out the cover sheet and attach it to the requested documentation.
  3. Fax (or mail) the documents for the College Board's review to the fax number (or address) on the form.
Supplying Additional Documentation
The system will indicate what, if any, additional documentation is needed. If you do not have the exact documentation requested, submit the paperwork that you have. At a minimum, it will need to show that:
  • The student has a disability.
  • The disability leads to a functional limitation in ability to take College Board tests.
  • The requested accommodation is appropriate.
Some examples of documentation that may assist in demonstrating the above are:
  • Psycho-educational or medical evaluation (full report)
  • Teacher survey forms
  • Outdated test reports along with information demonstrating the current impact of the student's disability
Notification
You will receive an email notifying you that a decision has been made. Check your dashboard to see the student's status or to read the decision letter. Parents or students will receive a letter by mail identifying the approved accommodations or explaining why accommodations were not approved.
Family Requests for Accommodations
Although it is recommended that families and schools work together to request accommodations, a parent/guardian may request accommodations without the participation of their school. For the paper application process, a student's parent/guardian will need a Student Eligibility Form — if the counseling office does not have the form available, have the family contact the SSD office. Please refer families to for more information. To have a decision in time for the test date, families need to submit the form well in advance.
