Parent Commitment Letter Agreement

My daughter,______, has been chosen for the ______team and accepts that position, she commits to participate in club volleyball for the 2015-2016 season with MOJO Elite Volleyball and understand the time commitment involved.

· I understand the club volleyball season is long and intense; I am committed to supporting my daughter, her teammates, and her coaches throughout the season in a positive manner.

· I understand practice is the core of the learning experience of each player in club volleyball. Tournament playing time will be based upon skill level, participation in practice, performance, effort, commitment, and attitude of the player during practice. All tournament playtime is at the discretion of the coaches. Tournament playing time will not be equal.

· I agree to submit payments on time as outlined on the Payment Schedule. I understand that if my daughter withdraws from a team, it does not exempt our family from our fiscal responsibility. Withdrawing after January 1 will require the remaining payments to be paid in full immediately. Withdrawing prior to January 1 will require a prorated amount be paid based on expenses at the beginning of the season. MOJO does not issue refunds, unless a season-ending injury occurs, which would be reviewed by the Executive Staff on a case by case basis (important documentation will be required). Please note that any player not in good financial standing with MOJO (including, but not limited to, outstanding payments for a club season, clinic, camp, league, etc.) will not be allowed to accept a position on a 2016 team until that standing is rectified.

· I understand that volunteering is a necessity for MOJO to run and I will contribute as I can throughout the season.

· I understand that major tournaments are regularly schedules on weekends and holidays; including Martin Luther King day, President’s Day, Easter (matches are played on Easter Sunday), and Memorial Day weekend. I am committed to my daughter’s attendance at all of these tournaments.

· I understand that if my daughter accepts a position on one of our top teams (18, 17-1, 16-1, 15-1, 14-1, 13-1), she will be required to participate in the Explosive Performance Conditioning Program.

· I understand that if my daughter or I have a problem during the season, I will follow the proper steps outlined in the MOJO Handbook to effectively solve the problem.
