Appendix A - Responsibilities

OVERALL RESPONSIBILITIES

Your activities have been defined by the Chair of the Department of DEPARTMENT. Including your clinical duties under your hospital appointment, as per your role of ARC CATEGORY ie:Clinician Teacher the approximate allocation of your time will be 60% Clinical Service, 25% Teaching, 5% Research, 5% Administration, 5% Health Care Leadership/Role Model/General Contributions. Your responsibilities as a Fill to what is appropriate ie: .5 FTE, Clinician Teacher are outlined in the Academic Role Category description at Appendix B. Clinical Service responsibilities are part of your hospital appointment.

ACADEMIC RESPONSIBILITIES

You will be expected to take part, as requested by your Department Chair, in aspects of governance, policy formation, curriculum planning, program implementation, surveillance and evaluation as carried out by the various committees of the Department and the University.

Attendance at Meetings: As a Clinical Academic faculty member in the Department of DEPARTMENT, you will be expected to attend monthly Departmental Committee meetings, Faculty Development sessions and Grand Rounds. As well, after the first year of your appointment, you will be expected to identify one of the Department’s standing committees on which you would wish to participate. Career Development Planning: Your performance will be reviewed on an annual basis. This process provides the Chair of the Department of DEPARTMENT, and yourself the opportunity to review your accomplishments and contributions to the Department, as well as discuss your short and long term career goals.(NOTE: If different for your Department please modify)

Departmental Practice Plan and Tithes: As a Clinical Academic faculty member in the Department of DEPARTMENT, you will belong to the Department’s Practice Plan and will be subject to an annual tithe of $XXX per year payable monthly. The Department reviews the tithe rate annually and is subject to change. You may be required by your Department, as per your Department’s practice plan, to set up a personal collection account, or utilize an existing collection account, if already set up at your INSTITUTION, for regular collection of tithe amounts. Should you wish, please contact the Department Chair to review the current Practice Plan. The claiming of an investment tax credit under Subsection 127(5) the Income Tax Act (Canada) in respect of the transfer of Tithes to the University by the Appointee in computing income tax shall be solely the responsibility of the Appointee.(NOTE: If different for your Department please modify)

Academic Health Science Centre (AHSC) Alternate Funding Plan (AFP) Agreement: Members of the Department of DEPARTMENT AFP Practice Plan, currently voluntarily participate in this agreement, which provides physician compensation funding from the Ministry of Health and Long-term Care for Physician Clinical Academic deliverables within the AHSC, as defined in the agreement. Should you choose to join this plan, you will receive from your AFP practice plan, clinical academic funding shared with other AFP practice plan members. Currently funding is approximately $XXX. per annum paid monthly, (Fill to what appropriate ie: 0.5 FTE). (NOTE: If different for your Department please modify)

Academic Support: The Department of «department» will provide you with a salary of XXXX annually for performance of your academic activities.

All full time members of the Department will be required to open a collection account through the Universityat the Bank of Montreal. An information package and application are enclosed in this offer. MENTOR / MENTORSHIP COMMITTEE

We are committed to providing you with the necessary mentorship in your new academic career. As per the Schulich Mentorship Program, we have developed a mentorship committee for you, as part of the Department of DEPARTMENT Mentorship Program. Your mentor / mentorship committee will include ENTER NAMES. The Chair of the Department or delegate will be in contact with you to set up the first committee meeting in the first few months of your start date. The purpose of the mentoring program is to assist and guide you in your clinical, teaching, and research role (where applicable) with the goal of reaching Associate Professor within 7 years. As per the Schulich Mentorship Program, you have the opportunity to engage in this process and we encourage it strongly.
