OSSAA Eligibility Checklist

OSSAA General Eligibility Requirements - Students must:

a.  Be enrolled in Mustang Public Schools

b.  Have on file an OSSAA Eligibility Record Form (7th, 9th and new student to district)

c.  Have on file a current OSSAA Physical

d.  Have on file current concussion management paperwork

e.  Have on file a current Parental Consent Form (flip side of physical form)

f.  Comply with all age/attendance/academic requirements set forth by the OSSAA

Questions to cover with your athletes:

·  Big Five A’s of Eligibility – Age, Attendance, Academics, Address, Amateur Status

1.  Is anyone new to Mustang Public Schools? (Address)

2.  Is anyone overage to participate at grade level? (Age)

14 Years-7th, 15 years-8th, 16 years-9th, 19 years any level

3.  Have you changed residences in the last year? (Address)

4.  Did you pass 5 classes last semester that count toward graduation? (Academics)

5.  Did you attend all classes 90% of the time last and current semester? (Attendance)

6.  Do you live with someone different than last year? (Address)

7.  Do you live in the MPS boundaries? With whom? (Address)

8.  Have you repeated a grade since 7th grade began? Which Grade? (Academics)

9.  Are you a resident of, or a transfer to MPS? (Address)

10.  Have you done anything to jeopardize your amateur status? Been paid to play? (Amateur)

*It is important that your athletes be honest when answering these questions in order to determine or maintain their eligibility status. Even if they have been an MPS student for a long time, their eligibility status could change depending on a variety of factors. You don’t need to single kids out when asking these questions, just address the group and have them approach you if they know they fall in one of the categories in question.

**Make sure that your athletes are in full compliance before any scrimmage or competitions begin. If you ever have any doubt, please contact me and I’ll look into it for you. It is my goal that when your competitions begin, you can give your full concentration to coaching!
