Open Enrollment Information

Dear Valued Employee,

The intent of this letter is to provide you with all the necessary information for this year’s open enrollment period, cover some important procedural information, and make sure you are aware of the options available to you as well as documentationrequirements. Even if you do not wish to make changes at this time, please read this message in it’s entirety as there are changes that affect everyonefor the 2011 plan year.ITTS also highly encourages you to verify that all of your desired insurance coverage / enrollments are in place andreview the Employee Benefits Booklet for further details on all items listed below.

Open Enrollment Dates/Times: The Open Enrollment period for Insurance Benefits will befrom 12:01am on 08 November through midnight 24November 2010. These times are in the Mountain Standard Time Zone so please adjust accordingly to the time zone at your location. Any changes not made (except for Qualified Status Changes) to medical and dental plans during this time period will have to wait until the next Open Enrollment Period in 2011, no exceptions.

Effective Dates: Newly elected Voluntary Term Life coverage in any amount will be effective on the first of the month following approval by CIGNA Group Insurance. All other policy changes will be effective 1 January 2011.

Enrollment Website: The new enrollment website is Yourlog on ID is your initials plus your clock number and your PIN is your date of birth (mmddyyyy). From the Enrollment Summary page, click the green “edit” button beside any plan you wish to change. To finalize your changes you must click the “Accept” button. Upon completion of the enrollment,it is advised that you either print or email yourself a copy of the enrollment summary by clicking on the button in the upper right hand corner. Employees without internet access will have the option of enrolling or making changes to their coverage by callingthe Employee Service Center at 1-877-976-2269 or +801-676-2260. The center’s hours are 8am-5pm EST Monday through Friday.

Dependent Eligibility Changes:Dependent children may now be covered on the medical and dental plans up to age 26. They do not have to unmarried or full time students to be eligible.

Documentation Requirements – Please read thissection carefully and ask for clarification if needed.

Dependent Documents: When enrolling any new dependents in your medical or dental plan, documentation supporting dependency by will be required by 15 December 2010.If documentation is not supplied to ITTS by this date, any dependents added during open enrollment will be removed from the policy. The next opportunity to add your dependent will be open enrollment at the end of 2011 with an effective date of 01 January 2012. A list of acceptable documents is available on the enrollment website.

All required documents should be emailed to or faxed to 1-877-722-9250 (US). Do not send them to the individual email addresses of your benefits representatives or mail these documents. If you need assistance with sending these documents, please see your local HR Representative PRIOR to the deadline. Delays caused by not following submission instructions will not be justification to extend the document submission deadline.

Social Security Number: Entry of a Social Security Number (SSN) or Individual Tax Identification Number (ITIN) for each dependent enrolled on your medical or dental plan is required,regardless of when you enrolled them. If your dependent is not a US Citizen, you will need to check the box to indicate this on the enrollment site. If your dependent is a US Citizen, and over the age of two, you will not be able to enroll this dependent without entering a social security number. Please be proactive and obtain this information prior to attempting to enroll your dependents.

Medical /Dental:PLEASE READ CAREFULLY:ITTS is changing medical and dental provider to Cigna International Expatriate Benefits (CIEB).If you do not wish to change your current medical and dental elections there is no action required. Your current AGB plans will be rolled over to the equivalent CIEB plan prior to 01 January 2011. However, you are strongly encouraged to check yourcoverage during Open Enrollment and make sure you are enrolled in the coverage you need/want.

AGB Catastrophic Plan (high deductible) = CIEB Open Access Plus (OAP) Catastrophic Plan

AGB PPO Plan (full coverage outside the US) = CIEB Open Access Plus (OAP) Global Plan

There are minimal changes to the annual deductibles and some co-pay obligations. Please review these changes on the summaries provided in the 2011 Employee Benefits Brochure.

A Welcome Kit will be sent to each employee. They will be shipped separately from the Employee Benefits Booklet. In the meantime, state-side and international providers can be viewed at You will need to Log in as the “Employer”. The temporary ID# is ITTS12011 and the Password is Cieb2011. When searching for providers outside the US, select “CIGNALinks Network”. All providers in this network have a direct settlement (direct bill) agreement with CIEB.

Once we go live you will be able to register for a secure account and be able to printtemporary ID Cards, review claim submissions, utilize the personal health assessment tools, and more.

RX Home Delivery Service: If you have an existing reoccurring prescription set up with AGB you can transfer this prescription once our files are set up in CIEB’s database. This should occur around 20 December. Detailed information for this process can be found on CIEB’s website.

Opting Out of Medical Plans:All those who are currently opted out of medical coverage will be automatically opted out againfor plan year 2011; unless you choose to elect a medical plan during the open enrollment period.

If you are opting out for the first time during the open enrollment period, you are required to submit proof of other coverage and the Opt Out Acknowledgement form prior to the close of open enrollment.The form can be located at and is included with the 2011 Benefits Brochure. All documents must be submitted to by midnight MST on 24 November 2010.

Vision

Vision coverage is included in the CIEB OAP Global plan and cannot be elected separately. Each covered member will be entitled to one eye exam every 24 months which is covered at 100%. Hardware is covered at 100% up to a $500 maximum every 24 months.The deductible is waived for both the exam and the hardware benefit.

CIEB Rates

Short Term Disability (STD): ITTS offers two plans for STD coverage. The base plan provides a weekly benefit up to 50% of your base earnings to a maximum of $500 per week. The second plan is a buy up option that provides a weekly benefit up to 60% of your base earning to a maximum of $750 per week. You have the choice of electing one of these plans; but not both.

A pre-existing condition clause will apply to both options. Employees who are currently enrolled in an STD plan that wasn’t subject to the pre-existing condition will be able to continue this exemption on the base plan. However, if electing a buy up option, the limitation will apply to the increased benefit portion.

Long Term Disability (LTD): ITTS offers two plans for LTD coverage. The base plan provides a monthly benefit up to 50% of your base earnings to a maximum of $5000 per month. The second plan is a buy up option that provides a monthly benefit up to 60% of your base earning to a maximum of $5000 per month. You have the choice of electing one of these plans; but not both. A pre-existing condition limitation is applicable on both plans regardless of when coverage is elected.

Voluntary Term Life Insurance:Any amount of coverage elected will require successful completion of the application process and approval by CIGNA. To begin the application, click on the link located in the enrollment portal and complete the Evidence of Insurability (EOI) form. You will be directed to enter a keyword. The keyword is ITTSENROLL. You will also be asked to provide an address in the US. This address will determine where the policy is written and has no effect on rates. Once the form is completed, it will be routed electronically to CIGNA. Notification of approval or request for additional information will come from CIGNA directly to your address on file in the payroll system; not the address you list on the form.

Addresses/ ID Cards:Please ensure your address in the payroll system is correct. Any changes to your address must be made in the payroll system through your HR Representative prior to the close of open enrollment. Changes to dependent addresses will need to be made on the enrollment website. Dependent ID Cards will be mailed to the dependent’s address in the enrollment system. Initial ID Cards for all employees will be sent to the Middle East Offices for further distribution to the field. Future communication from the carriers and ITT will be mailed to your home of record address.

Qualified Event (QE) -Please read this section carefully and ask for clarification if you don’t understand your obligation!Changes to medical and dental benefits due to a “Qualified Event” must be submitted to the benefits officewithin 30 daysof

the eventAND be accompanied by the supporting documentation.If you are having a problem obtaining the required documentation within this 30 day time frame, you mustnotify the benefits department PRIORto the deadline so we can informHeadquarters of the reason for delay. If you fail to meet these requirements, your next opportunity to make any changes will be during Open Enrollment in 2011, for a 1 January 2012effective date.For further details on what constitutes a “Qualified Event”, please review the “Participation Guidelines” in the Employee Benefits Booklet.

Your benefits are your responsibility. It is important that you make this a priority. Do not wait until the last minute.
