This form is to be completed by authors who choose to publish their article on an Open Access basis (i.e. the full text of the Article can be accessed by readers without charge).

Please read this form carefully and sign on page 2. If your employer owns copyright in your work, please arrange for your employer to sign where indicated.

Please return a copy of this form us as quickly as possible. You (or your employer, where appropriate) will remain the copyright owner of the Article.

We will not be able to proceed to publication until we have received a signed copy of both this Open Access License Form and the accompanying Payment Authorization Form.

You hereby grant Future Science Ltd an exclusive license to publish the above article (the “Article”) including the abstract in printed and electronic form, in all languages.
