Terms of reference

This award is presented to a pharmacist on the basis of outstanding service to the community. The recipient must:

  • be a pharmacist, licensed within the jurisdiction in which the award is presented
  • be living (awards are not presented posthumously)
  • not have previously received this award
  • not currently serving on the Membership Services Committee or as an officer of the Association (unless in ex-officio capacity)
  • have an outstanding record of community service which, apart from their specific role as a pharmacist, reflects well on the profession.

No self-nominations will be accepted.

Criteria at a glance

  1. Living pharmacist, licensed in Ontario
  2. Not a previous award winner
  3. Not a current member of the Association Board of Directors or Membership Services Committee
  4. A current Association member
  5. Outstanding community service record
  6. Community service activities outside of pharmacy, but which reflect well on the profession

Additional information about the nomination process:

The Ontario Pharmacists Association will contact the nominee to sign and return a waiver stipulating that:

  • they agree to be nominated
  • they are a member in good standing with both the Association and the Ontario College of Pharmacists (OCP)
  • they have not been engaged in any prior illegal or criminal activity and that they are not aware of any forthcoming legal claims
  • they agree to forward any additional relevant materials to support their nomination

The selection of awards recipients is made by the OPA Membership Services Committee and is subject to confirmation by the OPA Board of Directors. The awards will be presented on Friday, June 8, 2018 at the Awards Ceremony, which is part of the OPA’s annual conference.
