Clear photocopy of the Italian/EU passport of parent/s
7. / PROOF OF RELATIONSHIP
Original and photocopy of the applicant’s Birth Certificate issued by NSO and authenticated by DFA within six (6) months and formally translated in Italian language
Photocopy of Marriage Certificate registered in the Italian Municipality of Residence (Estratto di Matrimonio rilasciato dal Comune di residenza) or the transmission of registration of the marriage certificate to the Italian Municipality performed by the Consular Section. For spouse of other EU citizens, Marriage Certificate registered in their pertinent EU country.
For minors, Notarized Affidavit of Consent of the other Parent with attached valid document of identity. If one parent passed away, NSO death certificate legalized by DFA
In case applicant is descendant of the Italian/EU citizen (or of EU citizen spouse) and is older than 21 years of age, proof of dependency must be provided
