Dear Patient
On-line ordering of repeat prescriptions- Patient Access
We apologise for any inconvenience causedrecently whilst the previous system was not functional.However, we are pleased to inform you that this service is now available once more- using a secure system known as Patient Access.
Eventually, as well as ordering your repeat prescriptions, we will enablethis system to allow you to notify us of changes in your personal details eg address,to look at your recent blood test/investigationresults, to book/cancel appointments and look at your medical records. We will notify you in future as and when these options become available. However please note that currently the previous system for booking appointments on-line will remain in place.
If your repeat prescriptions are dispensed in weekly trays, this system is not for you. Please contact your pharmacy directly if you have any queries regarding ordering your trays.
How to register to use the Repeat Prescription Ordering service
Please take away a copy of this letter and then return it to the reception desk personally,having completed and signed the consent section, plusone of the followingoriginal ID documents
Photocard Driving LicencePassport
National Identity Card
orplus two of the followingoriginal documents with your name and address dated in the last 3 months
Statement from a bank, building society or credit cardUtility bill
Council tax bill
Benefits or State Pension notification letter
Birth Certificate (under 18s)
National Insurance Card (under 18s)
You must be aged 16 years or over and you cannot register on behalf of another person.
The receptionist will then give you a letter containing the following 4 items:
- PIN
- Practice ID
- Access ID
- NHS number
Please then go online to register to use Patient Access via one of the following 3 methods :-
- Via a link from click on Repeat Prescriptions
- Via the website
- By downloading the free app Patient Access to your Smartphone.
Please note this App is not available for iPad.To use an iPad you must use the Safari browser to visit the above websites.
Click on Register and enter the 4 items from your letter when prompted. You will then be asked for your Surname, D.O.B, and you will be prompted to choose a password and enter your email address and/or mobile number. We will use these to contact you if you forget your User ID or Password to reset.
Finally, you will be asked to complete 2 security questions.
Once registered, please note down your own User ID and Password for future reference. You will be sent an email confirming that you are now registered to use Patient Access
You will now be able to use your User ID and Password to Sign Inand follow instructions to make a request to order your repeat medications.
IMPORTANT –YOU MUST ADVISE US IN THE FREE TEXT BOX IF YOU WANT TO PICK UP YOUR PRESCRIPTION FROM OUR HALTON, BOLTON-LE-SANDS OR SILVERDALE SURGERIES, SO WE CAN PRINT YOUR PRESCRIPTION THERE.
Please allow 2 working days for your prescription request to be processed before collecting.
