Oklahoma Transition Institute Team Goals Tool

Use this tool to document your team’s transition goals for the coming year. Some goals have a more rapid implementation than others, so this may be a short term, or a long term plan of action. The action steps are the primary steps that need to be taken in order to achieve the desired goal. It is desired that a Transition Team have two to three goals for a program year, however in some cases it is a big task to get even one new program, or change in place for a school site. Building relationships!!!
1. Transition fair (BA, Jenks, Union)/check with TCC/Lonnie to see if they are withdrawing from the process. If Lonnie drops look at having at JPS. (Robert checking on)
2. Reaching Start early/elementary schools/pamphlets, information, increase parent involvement
3. website
4. Successful employment outcomes,
5. Training elementary teachers to involve OKDRS staff/professional development meetings/give out DRS folders to teachers to give out at IEP meetings.
6. Give out information at department head meetings for all schools
7. Phoenix Rising (released from prison, homeless)/resources available for clients, no role models, job sites, constant support
8. Develop information to present to teachers/overview pamphlet
9. DRS come to school to do PD meeting (Ramona set up)
10. Esther Watkins – elementary information
11. Information email: parents rights, lnh, parent survey, voc rehab app, drs folder. Have caseload teachers email out prior to IEP meetings.
12. Meeting prior school and look at all sped kids see who to target.
13. DRS serves 504 kids that have an ongoing medical issue (chronic)
14. Have Esther invite Bixby to next meeting.
