WORK HISTORY:

Perform necessary health assessments to determine appropriateness of care coordination and case management services for all identified members. Review the member’s utilization and pharmacy profile and outreach to coordinate PMP, specialty providers and the member for appropriate treatment plan to optimize access to and utilization of health maintenance and restoration of health intervention. Educate members on the care management/case management process and scope of services offered. Educate members on the understanding of their disease and any medications, interventions, or behaviors important to achieving good outcomes.
