NYSAN Program Quality Self-Assessment (QSA) Tool- Action Planning for Improvement Tools

Instructions:

Use the NYSAN Program Quality Self-Assessment Tool: Program Improvement Inventory with your focus group or improvement team to put together a comprehensive list of areas in need of improvement. This tool can be filled out to include information on all QSA elements that the group is self-assessing. Filling out this tool will help your group recognize and select improvement areas of the highest priority for focus. (See the example for ideas on how to fill out this template).

Some questions to consider when prioritizing:

What relates to safety?

How does this relate to our mission?

What will impact quality the most?

What will impact quality for participants the most?

How long will it take?

Who will be responsible for working on this?

Who do we need to involve?

Does something else need to be done/decided before we can do this?

What type of support do we need?

Select areas of focus, and then complete the Action Plan for Quality Improvement for each QSA element that will be focused on for improvement. Each Action Plan for Quality Improvement may focus on multiple indicators and include multiple goals, but should only represent one QSA element. (See the example for ideas on how to fill out this template). Be sure to designate the Point Person/Facilitator who will be responsible for following up with the person accountable for each step in the improvement plan.

Participants noted that they did not feel supported academically, and did not receive homework assistance unless they specifically asked for it.
Parents requested that students have time to work on their homework during the afterschool program.
Change Needed: Staff will effectively support participants in understanding and working on their homework. Designated time during the program schedule will be added for participants to work on their homework, with additional time offered for those participants who wish to continue working on their homework past the allotted time. Optional ELA tutoring will be offered for those participants who wish to take advantage of this service.
