- New faculty must complete the Faculty Form(attached page 5)and send to the Nursing Education liaison via email or postal mail.

- Complete the Orientation Packet and Faculty Guidelines located on The Clinical Coordination Partner (TCCP) webpage. This needs to occur in advance of orienting to the nursing unit and the start of clinical experience.
- Attend MCHS-FH Faculty Orientation. Offered August of each academic year.
- Contact the hospital pharmacy for pyxis access. 608-392-2358
- Go to Security office to obtain parking passes. Be sure to get enough for your group of students.
- Located on the 1st floor of the St. Francis building. 608-392-2956
- Have photo taken for ID badge. All faculty must wear a MCHS issued badge and a school badge at all times while on campus.
- The photo studio is located on the 5th floor of the Professional Arts building, Room 5-47, and is open on Mondays from 7:30-8:30 a.m. and 11 a.m.-12:30 p.m.or by appointment.

- Be sure to include the last 4 digits of your social security number on the photo application form. This number will be your access code pin. If the sequence begins with a “0”, replace with a “1”.
- Badge will be ready for pick up the next day in security.
- All students with clinical experience in the St. Francis, CAMS, or Sparta Hospital buildings must have a MCHS-FH issued access badge.
- For La Crosse campus, pick up badges from the MCHS-FH Nursing Education Liaison @ 608-392-9402
- For Sparta campus, contact Sparta Admin Office Associate @ 608-269-2132
- If needed, make conference room reservations for post-conference and orientation day through Jill Lorenz, Admin Assistant, 608-392-4443
- Send the department managers a list of students who will be on the floor.
