Notification to Student of Issues to be Addressed

An area/areas of your performance on placement has been identified as not yet satisfactory.The table below has details of the performance area/areas to be developed, and the evidence of improvement required.

These improvements must occur within the next four placement days. You will be reassessed at that time, and if the necessary improvement has not been demonstrated, your placement is at risk, and may be cancelled. Your Agency Supervisor will meet with you at the end of each of the four placement days and provide verbal or written feedback on your performance that day in these areas. This is intended to support you in developing the required skills / attitude / professional behaviour.

This notification will be emailed to you following this meeting and Cc’d to your Placement Coordinator. Your Clinical Supervisor will then be informed so support can be provided throughout the period of development. Please contact your Placement Coordinator should you wish to discuss the issue further or request support.
