SECTION II: OBJECT OF THE CONTRACT
II.1) DESCRIPTION
II.1.1) Name given to the contract by the contracting entity:
SUPPLY I INSTALLATION OF A COMPLETE SYSTEM OF SOLAR COLLECTORS
II.1.2) Type of contract: construction work
No right of appeal.
II.1.3) Description of the object, size and scope of the contract:
The object of the order is to supply and install a complete system of solar collectors and carry out construction work connected with its installation and to adapt the hitherto boiler room (heat exchanger room) at the Nursing Home – Dom Pomocy Spolecznej, ul. Dojazdowa 5/7, Lodz.
Specific scope of work:
1. Assembly of a structure to support the collectors.
2. Renovation work in the heat exchanger room.
3. Construction work regarding electrical installations.
4. Dismantling work.
5. Solar installation;
a. Assembly and installation works,
b. Anticorrosive works,
c. Insulation works,
d. Concurrent works.
Detailed scope of work and a list of materials necessary to complete the contract have been determined in the project documentation enclosed with the SIWZ (Specification of Valid Employment Conditions) and the bill of quantities. .

II.2) DURATION OF THE CONTRACT OR TIME LIMIT FOR COMPLETION: period in months: 3 from the award of the contract.
SECTION III: LEGAL, ECONOMIC, FINANCIAL AND TECHNICAL INFORMATION
III.1) CONDITIONS RELATING TO THE CONTRACT
· Information about tender guarantee: Contracting entity demands a tender guarantee of 4000 PLN
III.2) CONDITIONS FOR PARTICIPATION
· Description of terms conditions for participation in the procedures and description of the evaluation of fulfilling the conditions:
Contractors who fulfil conditions determined in item 22, section 1, subsections 1-3 and not excluded on the basis of item 24 of the Act, may apply for the contract award.
1) The contracting entity shall regard the condition determined in item 22, section 1, subsection 2 as fulfilled, in relation to the contractors who:
- have employees who, in accordance with the requirements of item 12 of the Act of 7 July 1994 ‘Construction Law’ (uniform text, Journal of Laws of 2003, No. 207, clause 2016 later amended), may act in an independent technical capacity in construction work, including construction work management as described in the contract, and hold required qualifications along with current certificates of membership of an adequate chamber of professional authority, in the following specialties:
a) Construction and building,
b) Installation regarding thermal installations,
c) Installation regarding electrical installations
2) The contractor shall regard the condition determined in item 22, section 1, subsection 3 as fulfilled, in relation to the contractors who:
-have financial means or credit standing no less then 75% of the tender value (gross).
The evaluation of the fulfilment of the aforementioned conditions shall be carried out in accordance with the ‘fulfils – does not fulfil’ formula, on the basis of the information provided in the documents or statements specified below.
The contracting entity demands that the contractor awarded the contract used in the part financed by EkoFundusz (i.e. 160,279,00 PLN net) suppliers or contractors from the USA, Switzerland, Norway and/or EU states.
· Information about statements and documents which shall be provided by the contractors in order to confirm the fulfilment of the conditions for participation in the procedures:
1). In order to confirm that the contractor is authorized to perform particular activity or action and is not excluded on the basis of item 24 of the Act, the contracting entity demands the following documents, either in their original form or as a photocopied attested copy:
1. The contractor’s statement about the fulfilment of conditions of item 22, section 1 of the Act, in accordance with Annex No. 2 to SIWZ,
2. The contractor’s statement about not being excluded from the procedures in accordance with Annex No.3 to SIWZ,
3. current duplicate from an adequate register or a current statement about the entry to the business activity register, if separate regulations require the entry to the register or notification about the business activity, written out no earlier than 6 months before the deadline for submitting applications for participation in the procedures for contract awarding or submitting bids;
4. current certificates by an adequate head of the Revenue Office and the adequate department of the ZUS (Social Insurance Institution) or KRUS (the Agricultural Social Insurance Fund) confirming respectively that the contractor’s taxes, fees and insurance contributions do not remain unpaid, or certificates that they have gained legally valid exemption, adjournment or arrangement of instalments for the payments due or total stay of execution of the decision of an adequate body – written out no earlier than 3 months before the deadline for submitting applications for participation in the procedures for contract awarding or submitting bids;
2) In order to confirm the condition described by the contracting entity of the contractor having the necessary knowledge and experience and having the technical potential and persons able to carry out the contract, the contracting entity demands:
1. documents stating that persons who shall be participating in carrying out the contract, hold adequate qualifications, if they are obliged by legal acts to hold such qualifications (i.e. suitable qualifications for the aforementioned employees and current certificates of membership of adequate chamber of professional authority).
3) In order to confirm the condition described by the contracting entity of the contractor being in an economic and financial situation which ensures carrying out the contract, the contracting entity demands the following document:
information from a bank or a credit union where the contractor has an account, which confirms their financial situation or their credit standing written out no earlier that 3 months before the deadline for submitting applications for participation in the procedures for contract awarding or submitting bids;

In the aforementioned proceedings, any applications or notifications shall be submitted by the contracting entity and the contractors in a written form.
