Notes on the revised consultant timesheet and service report

Ø  This combined time sheet and service report replaces our separate timesheet & job report with effect from April 2012 and should be completed at the end of each month and also at the end of the job itself.

Ø  In particular we want to capture specific job information regarding tasks and equipment used; in order that we can routinely update your CV details and career history and make you more attractive to future clients.

Ø  Clients and Consultants will only be asked once to comment on any job completed.

Ø  If a job spans more than one calendar month you may use this form as a timesheet only until the end of the job when the full appraisal and job report sections should be completed on the last sheet submitted.

Completing the form

·  Please complete this form by inserting your NAME in the green box

·  Insert the month, Year, Client Name (e.g. Schlumberger/Repsol etc) and location (e.g. North Sea/China etc)

·  The date column is numbered 1-31 to cover every day of the month - if you travel to the job on the fifth day of the month, start the Service Report on row #5

·  Enter a “T” on days on which you travelled and a “W” on days on which you worked

·  Well number is helpful for billing the client and can speed up payment

·  Location: Please put rig name here, again it speeds up payment processing

·  Description of Duties: Examples; Flow Well, Maintain Compressor, run in Coiled Tubing, WOW, Standby, Travel to Rig etc

·  IMPORTANT: Please get your supervisor to tick off the appraisal feedback at the end of the job (Appraisal, is however not required if this is the month end timesheet in the middle of a job)

·  If possible get them to say you did a great job in the comments box

·  Ideally you should get this done in private, or leave it with the Supervisor to complete in private if there is a room full of people. All companies are focusing on proof of competence, and the best proof is previous good appraisals.

·  MOST IMPORTANT: Get your timesheet signed by someone in authority directly employed by the client whenever possible, preferably the Location Manager. Failure to get this signed off WILL delay your payment until we get approval from the base manager.
