Notes from the Avenue Patient Forum meeting held on the 24th September 2015

Matters arising/Outstanding:
·  Bereavement Information Sheet - a contact provided by GJ, has been added to the sheet. It was agreed not to add’ Merry Widows’. NC is sure some of the ‘0845’ numbers could be replaced with local ones. He will advise AL who will make the amendments.
·  Name Badges for staff - remains outstanding.
·  Cirencester Hospital pathology opening times – AL was going to contact the Hospital as these did not appear on their website. As this was proved to not be the case, no further action was taken. Sian will pick this up individually if she deems necessary.
·  The Prime Minister’s Challenge Fund - Clinics due to open on 19th October. As yet, no further information available.
·  New Practice Website – Provided by ‘mysurgerywebsite’. Examples of their work can be found on: www.thephoenixsurgery.co.uk, www.pricemills.co.uk, www.ridgewayviewfamilypractice.co.uk. Please critique the new website when it goes live next month. Comments to AL.
·  NC offered to produce a new PPG membership poster for the patient waiting areas. / NC

Friends & Family Test:
The results for August 2015 were tabled and discussed:
·  The results remain very positive and only a very small proportion of patients would NOT recommend us.
·  As the results do not to vary much, month on month, it was agreed not to spend too much time on them. AL will bring any relevant comments etc to the group’s attention, as and when necessary.

Practice Leaflet:
·  The Draft printed version of the leaflet was tabled and discussed at length. A number of amendments were made, (which AL will pass onto the printers). / AL
Dementia Event:
·  NC has contacted the Alzheimer’s Society re the local branch providing/manning the vehicle. NC to chase if he doesn’t hear back.
·  GJ/NC visited the William Beveridge roadshow recently held in town. They do a lot of work in Bangladesh and the UK. They are happy to be involved in our event. NC has the contact details when needed.
·  AL needs to contact Bill Wragge at Cotswold District Council re permission to park the van in town.

·  Still no inspection date for the Practice!
Practice News:
·  We are pleased to report Carole Cook has recently returned to the Practice as Medical Receptionist.
·  Work on the main patient waiting room continues with new windows, chairs and flooring.
·  The Practice will pilot a software package called ‘askmygp’ (enabling patients to email questions to GP, rather than making an appointment).
Patient concerns:
·  No formal complaints received
·  NC aware of flu reminder texts being sent but no flu clinics available to book online. This has now been rectified.
Joint PPF Meeting:
·  Next one planned for 2nd November 2015.

·  The group remains keen for AG to attend a meeting within the next 6 months. AL will try to organise, bearing in mind, he now works elsewhere on his days off (Tuesdays & Thursdays) The Group is happy to reschedule the time/date of the meeting to accommodate this.
·  BH is concerned she is unable to use the Bariatric scales in the treatment room on an ad hoc basis. AL to see if there is a way of accommodating this without disrupting clinics or putting pressure on staff. / AL
