  1. Supervisors MUST investigate, complete and process an Incident report for all incidents, which occur within his/her area of control.
  • Incidents include Injuries, Property Damage, Dangerous Occurrences, Criminal, Environmental, Motor Vehicle and Equity Matters
  • If an injury is sustained, the supervisor must complete an “Incident and Injury Report” on TEMS. The details from the incident report form will transfer common details to the injury report. The supervisor will be required to complete the incident report as a part of the on line incident report registration.
  • Enter N/A if a section is not relevant to the incident.
  • When the report is submitted by the supervisor an email alert is forwarded to the Business Manager. Once the Business Manager has reviewed the report and is satisfied with the investigations and actions, the Business Manager will use the “Save and Notify”button. The notification matrix is built into the on-line system therefore notifying all relevant Managers and Directors.

Note: There is no need to email the report to key stakeholders

  1. In the event of a major incident the Business Manager and Regional Manager must be advised immediately by phone. In addition, the online incident report is to be completed prior to the end of shift. The Business Manager shall review and submit for notification as soon as possible. A Major Incident includes but is not limited to the following occurrences:
  • Fatality;
  • A Government Authority is required to be notified immediately by phone;
  • Non disturbance occurrence which requires the scene not to be disturbed refer Section 1.5 CEVALogistics Risk Management Manual;
  • Members of the public or contractors are injured and require medical attention;
  • Equipment or material damage likely to exceed $5000;
  • Any significant damage or potential damage to the environment;
  • Any criminal occurrence.

Note:If a Major Incident requires telephone notification to key stakeholders in accordance with the notification matrix contained in section 6 of the CEVA Logistics Risk Management Manual, this telephone notification is still a requirement under the online reporting system

  1. For all other incidents the report must be completed and forwarded to the Business Manager and other required recipients within 24 hours of the incident.
  1. The Business Manager will, within 2 business days of the incident:
  • Review and take any necessary remedial action;
  • Complete the form electronically and obtain signatures on a hard copy (hard copy to be retained in branch);
  • If necessary, complete any Government Notification Forms and Workers Compensation Forms (refer section 6.5 of the CEVA Logistics Risk Management Manual to identify if Government Notification is required).
  1. The Business Manager will, within 7 business days of the incident:
  • Ensure a Task Analysis and subsequent SOP or review of an existing “Task Analysis” and SOP are undertaken for all work flow / injury related incidents and appendto the online incident report.
  • Ensure all criminal damage / theft related incidents that require a Work Place Inspection and / or Security Assessment to be completed and append to the online incident report.
  1. Business Manager will then:
  • File the original hard ‘signed’ copy for at least 7 years (incident reports and records detailing employee injury/illness should be retained for a period of 30 years);
  • Forward a hard copy to management team and OHS&E committee for further review/action;
  • Monitor the completion of any corrective actions of the incident report (section 5).

  1. The online Incident reporting provides for:
  • A requirement for the BU manager to close each corrective action prior to closing incident report
  • On-line update from key stakeholders communicated to BU Manager
  • Ability to group, analyse and trend by region, incident type, contract type, customer
