4. BASIC WORK SITE PROCEDURES

List of procedural steps and guidelines to be followed to ensure that the work is performed safely.

4.1 Work Site Method Statements

A listing of the appropriate method statements for each of the tasks to be performed, where a method statement may be defined as a document that details the way a work task or process is to be completed. The method statement should outline the hazards involved and include a step by step guide on how to do the job safely.

4.2 Incidents

The manner in which incidents will be recorded and reported and the forms to be used.

4.3 Risk Assessment

The procedure to be followed for conducting risk assessments and the form to be used for this purpose.

4.4 Equipment Inspection

The responsibility for equipment inspections, the frequency of inspections, the labelling of equipment and the recording of the inspections will be specified in this section.

4.5 Training Records

This section deals with training requirements to develop specific skills for specific tasks, the scope covered in training, the medical and psychological requirements for training and the keeping of training records.

4.6 Evaluation of Medical and Physiological Fitness

This section deals with the medical and psychological requirements (if applicable) for personnel who will be performing the work. It must also specify the forms and records to be generated and made available.

4.7 Emergency Preparedness and Response Procedure

This section deals with the procedures to be followed and training to be conducted in terms of emergency preparedness and response.

4.8 Inspection Records

This section specifies the different types of inspection records that will be required to be kept for the relevant equipment.

4.9 Enforcement of Fall Protection Plan

The person responsible for the enforcement and implementation of the fall protection plan must be appointed and be fully competent for the task. The requirements for training and competence must also be stated.

The responsibilities must also include the communication of the fall protection plan to all affected parties and a record of this communication must be made available if required.

5. RESCUE PROCEDURES

All work to be performed in order to complete the work and to initiate a rescue if required, will be in accordance with the appropriate standard.

All personnel who are to perform any work at heights are to have the necessary training in rescue techniques – this training must be current.

All rescue procedures shall be performed by competent persons in accordance to the relevant standard.

All the necessary rescue equipment must be on site including a first aid box. A list of standard rescue equipment must be listed.

A mobile telephone in working order must also be available on site to contact the local emergency services in case of an accident.

Emergency telephone numbers must be listed.

A list of different types of rescue procedures that can be performed either separately or in a complex combination of the manoeuvres according to the various emergency situations, must be listed.

Any other notes regarding the successful execution of the fall protection plan must be listed.

6. INCIDENT INVESTIGATION

This section lists the procedure to be followed for the reporting and recording of different types of incidents (e.g. minor or major), the forms that need to be completed and the procedure to be followed for the investigation of the incident and the correction steps and steps to be followed to prevent a recurrence of the incident.

7. CHANGES TO THE PLAN

This section stipulates the protocol to be followed for making any changes to the fall protection plan.

8. MONITOR AND REVIEW

8.1 Monitoring

This section deals with how and by whom the work in progress will be monitored and the follow-up action that may result from feedback resulting from this monitoring.

8.2 Review

This section deals with reviewing process for the fall protection plan, e.g. the review frequency, the inputs needed for the review, the parties involved and the resulting follow-up action.

The procedure used for the keeping of fall protection plan records, including communication records, will be stated here.10. Amendment History
