*During the period of April 1, 2013 – July 31, 2013, the Division of Vocational Rehabilitation Services (DVRS) Independent Living Rehabilitation Program (ILRP) provided services to 3,312 individuals with significant disabilities of which 263 individuals received personal assistance services, 14 individuals were deinstitutionalized and 35 individuals were prevented from institutionalization.

Notes:

·  Provision of services (n=3,312) means the case is in an eligible status (status 60 and above) on the last day of the time period or was closed from an eligible status during the time period.

·  Receipt of PAS (n=263) means a case with payment for PAS (case service code 27) during the time period.

·  De-institutionalization (n=14) means a case that was closed in status 76 from status 61 during the time period.

·  Prevention of institutionalization (n=35) means a case that was closed in status 76 from status 62 during the time period.

*DVR ILRP provided the transition coordination for 13 participants through the Money Follows the Person Project (MFP) for the period of April 1, 2013 – July 31, 2013.

*The Displacement Prevention Program received 459 applications. Of these 437 were approved for reimbursement. Currently 213 DPP requests for reimbursements have been received which amounts to $786,342.00.
