NJB Fingerprinting Process & Procedures

NJB has an exclusive contractual arrangement with L-1 Enrollment Services Division for fingerprinting services. NJB has worked with L-1 for fingerprinting in some capacity since 2004 to meet the growing needs of our agency. L-1 supports the safe environment requirements of the programs and will be in attendance at the scheduled clinics throughout the season to offer a convenient service to our valued chapter volunteers and affiliates. Coaches fingerprinted prior to the 2008-2009 year will need to have L-1 fingerprint them.

If you are unable to make one of the clinics where fingerprinting is offered and you need to make an appointment, you can do so by calling 1-800-315-4507 or schedule an appointment online at Appointments are necessary. Please remember to bring your form, ID and fees at the time of fingerprinting.

NATIONAL JUNIOR BASKETBALL ACCEPTS NO RESPONSIBILITY FOR INDIVIDUALS NOT USING L-1 Enrollment Services Division.

STEP 1: OBTAIN A FORM. All applicants submitting fingerprints must have aRequest for Live Scan Service form. Please click on the link below to access this form and print 3 copies.

STEP 2: PRINT THE FORM. You will be required to bring in a COMPLETED live scan form in order to be fingerprinted. This includes fingerprinting at clinics or at one of the approved L-1 offices. Please print the form from the link above and complete the information required.

STEP 3: COMPLETE THE FORM. Most of the information needed is already pre-printed on your form. You will also need tobe sure to complete the middle section of the form with your person data. Please remember to include the appropriate OCA or Chapter Code as well.

The live scan form should always be completed with the following information:

STEP 4: FEES. The fee for service is $17. Only CHECKs or MONEY ORDERS will be accepted on mobiles or at the scheduled clinics. NO CASH OR CREDIT CARDS WILL BE PROCESSED AT CLINICS OR MOBILE SESSIONS. Please plan accordingly.

STEP 5: PHOTO IDENTIFICATION. Everyone being fingerprinted will be required to show a valid photo ID to the fingerprint technician. Acceptable forms of ID include;

  • Non expired, state issued driver's license
  • Non expired, state issued identification card
  • Military I.D.
  • Non expired passport

PLEASE NOTE: When you have completed your live scan, the technician will fill out the bottom section or you live scan form. This information will include an ATI (or applicant tracking identifier) which is unique to your scan.
