The post holder will have continuing responsibility for a defined caseload and provide direct clinical care to patients and support to families with highly complex needs where there is a diagnosis of cancer or other life limiting condition. The post holder will provide clinical expertise in symptom management and specialist palliative care, support and counselling to patients who are suffering from cancer and their families.
Other duties of the role include acting as a specialist palliative care educational resource to all members of the multi-disciplinary team in a defined locality, contributing and collaborating in the delivery of evidence based care. The post holder will enhance the provision and development of palliative/cancer services within NHS Lanarkshire and across all health care settings in the attainment of the highest standards of clinical care
The post holder will provide bereavement support for carers and specialist advice to other professionals and agencies on bereavement related issues.

4. SCOPE AND RANGE
The service provides specialist palliative care nursing across Lanarkshire which has a population of approx 500,000 people, 92 GP practices, 3 community hospitals and numerous nursing / care homes.
Referrals are made for patients with cancer who have complex care needs requiring the support of a specialist palliative care nurse practitioner.
As an autonomous practitioner the post holder will function as a specialist resource to primary care teams in a defined locality and to the wider multi-disciplinary team which includes Marie Curie, community nursing services, acute hospital services, St Andrews Hospice, the Dalziel Centre and the Haven.
The post holder will assume delegated responsibility through referral for the specialist assessment, planning, implementation and evaluation of patients care needs.
The post holder will contribute specialist palliative care knowledge to the development and implementation of education programmes, local guidelines and policies.
MAIN DUTIES/RESPONSIBILITIES
Summary of main duties.Clinical
· The post holder is responsible and accountable for the management of a caseload of patients with complex cancer or specialist palliative care needs ensuring the holistic assessment of care needs, development, implementation and evaluation of specialist care packages in partnership with the patient/carers/family/multi-disciplinary team. Care plans will require adjustment and updating as the patient condition changes. This is complicated by the need to interface with other departments and organisations including acute, local authority and voluntary sector.
· Facilitate close collaborative working practices with Clinical Nurse Specialists and other health care professionals within Primary Care/Acute hospital setting and hospices in the promotion of holistic and seamless care. This includes attendance at appropriate clinics.
· To build and maintain a therapeutic relationship with the patient and his/her family, both in the home and during any periods requiring in-patient care.
· To act as a resource in providing specialist nursing knowledge, expertise and leadership to the multi-disciplinary team involved in caring for patients with malignant disease and non-malignant palliative conditions.
· Provide bereavement support including risk assessment and counselling to families and carers
· Be fully compliant with the NMC Code of Professional conduct and other relevant guidance documents, which support and maintain standards of professional practice for nurses and midwives.
· Ensure safe working environment in accordance with health and safety legislation.
· Take responsibility for personal and professional development in order to enhance knowledge, skills and values needed for safe and effective practice.
· The post holder will be required, with great accuracy, to administer medications including controlled drugs, orally, by injection or using mechanical syringe pump.
Research and Audit
· Participates in research or audit projects relevant to clinical area, disseminating and implementing relevant findings to improve patient care and treatment.
· Encourages and supports active involvement in research/audit and identifies areas for development in cancer/palliative care within own clinical area.
· Actively contributes to and on occasions may lead on the development, implementation and evaluation of evidence based policies, procedures and guidelines within the service. Negotiate with other members of the multi-disciplinary team (eg. GP’s, District Nurses and Community Pharmacists) where changes impact on them.
Education
· Actively contributes to the development, delivery and evaluation of specialist palliative care education programmes for health and social care professionals and other agencies.
· Identifies and responds to educational needs and requests from primary care colleagues on an ongoing basis including organising and facilitating study days and workshops to meet a wide range of education needs.
· Develop and deliver education programmes at local University at pre and post -registration level eg degree and post graduate level.
· Participates in the induction, orientation and mentorship of new staff.

Physical
· Minimal physical effort required during working day. Visiting clients in their own homes irrespective of domestic circumstances which may include unpleasant conditions/environment.

· Daily sitting in a restricted position while inputting data via a keyboard.
· Potential daily exposure to uncontained body fluids eg. Sputum, blood, tumour exudate, urine, faeces.
Mental
· Work is of an unpredictable nature which requires frequent episodes of intense concentration eg interpreting information/results or observing and interpreting information from patients/carers in distressing situations. Responding to rapidly changing clinical situations and the need to re-prioritise as a result of this. Continual need to manage complex emotional issues of patients’ and relatives.

· Daily exposure to highly distressing/demanding emotional and complex circumstances including breaking bad news, communicating with distressed patients, relatives, carers and caring for dying patients.

Managing and prioritising competing demands from patients, carers and other health care professionals whilst ensuring an efficient, effective , safe, responsive and flexible service is delivered to meet the rapidly changing needs of people facing advanced life-limiting illness.
No automatic cover for annual leave within team therefore additional burden when team reduced in numbers.
11. KNOWLEDGE, TRAINING AND/OR EXPERIENCE REQUIRED TO DO THE JOB
· Registered General Nurse with a minimum of 5 years post registration experience;
· Educated to/working towards/operating at Degree Level
· Evidence of post basic study in oncology/palliative care and evidence of continued personal development in that specialism;
