I/We assume all risks and hazards incidental to such participation including transportation to and from and do hereby waive, release, absolve, indemnify and agree to hold harmless the league organizers, coaches, sponsors, participants and persons transporting our child to and from activities of any claim arising out of an injury to our child

If called upon to produce proof of grade/age, participant will supply such proof, i.e. certified birth certificate. Falsification of such information can and will be grounds for dismissal of the above registered participant.
