Please complete the following regarding any international, faculty or staff applicant, if applicable.

  Contact information: current address, phone (including cell) and email address

  Current immigration status? e.g., H-1, H-4, F-1, J-1, etc. OR a permanent resident petition is pending

  Please attach copy of passport picture and date of validation page, and most current visa page for the professor and any accompanying dependents

  Please attach copies of both sides of the form I-94, a small white card, usually in the passport for the professor and any dependents

  Please attach copies of all related documents related to the current status (I-20s, DS-2019s, I-797s, etc.)

  Currently on F-1 Optional Practical Training (Please attach a copy of the EAD)

  Permanent resident petition is pending and I have a valid EAD (Please attach a copy of the EAD)

  Current position and place (name and address) of employment?

  Proposed termination date of the international employee’s current position?

  What is the proposed start date of your position at Texas State?

  Was he or she ever in J-1 or J-2 status and have you satisfied the 2 year residency requirement or do you have a waiver for same.

  Plans to travel* outside the U.S. : Please provide destination(s), date of departure and date of return

  Does the international professor have any non immigrant dependents? If so please list their names, dates of birth, relation to you and current immigration status.

  Do dependents have any travel* plans? Please provide destination(s), date of departure and date of return.

  Posting of the Texas State position in national print journal (Please provide a copy of the posting and copy of a page of the journal that indicates the date and name of journal.)

*Please note that depending on the nature of your petition, it may be necessary to postpone or cancel travel plans.
