NOTE: / If you are a CECAS user, please enter this information in the Student Information Form in CECAS.
LEA/Charter School and Number / Enter the name of the LEA or Charter School and the LEA/Charter number assigned by the state (ex: 490 or 49A).
Date / Enter the date the form is completed.
Student Full Name / Enter the student’s full name.
IEP Dates / Enter the dates of the current IEP.
Date of Birth / Enter the student’s date of birth.
Race/Ethnicity / Enter the student’s race/ethnicity as entered in PowerSchool.
EC Category / Enter the student’s area of eligibility for Exceptional Children services (ex: SLD).
English Language Learner / If the student is identified as an English Language Learner, enter “Yes.” If not, enter “No.”
Grade 2014-2015 / Enter the grade the student was in for the 2014-2015 school year.
School / Enter the school the student attended for the 2014-2015 school year.
Service / Enter the service the student received.
Note: The next four categories will use data from the second semester of the 2014-2015 school year.
Days Absent / Enter the number of days the student was absent for the second semester of the 2014-2015 school year.
Days Suspended / Enter the number of days the student was suspended for the second semester of the 2014-2015 school year.
Incidents of Violence / Enter the number of incidents of violence reported for the student in the second semester of 2014-2015 school year.
Hours in School / Enter the number of hours the student was in school for the second semester of 2014-2015 school year.
Note: the next two categories will use data from the 2013-2014 school year.
Math Test Score / Enter the student’s numeric score for the 2013-2014 end of year test in math.
Reading Test Score / Enter the student’s numeric score for the 2013-2014 end of year test in reading.
Note: the next six categories will use data from the second semester of the 2014-2015 school year.
Language Arts / Enter the student’s numeric grade for the second semester of 2014-2015 school year.
Social Studies / Enter the student’s numeric grade for the second semester of 2014-2015 school year.
Science / Enter the student’s numeric grade for the second semester of 2014-2015 school year.
Math / Enter the student’s numeric grade for the second semester of 2014-2015 school year.
P.E. / Enter the student’s numeric grade for the second semester of 2014-2015 school year.
Elective / Enter the student’s numeric grade for the second semester of 2014-2015 school year.
