NOISE

Are noise levels related to the event, including bump in, construction & performance, excessive?
85db exposure for 8 hours, 91db exposure for 2 hours, 106 db exposure for 3.8 mins, a maximum peak of 140db (C) {A copy of Worksafe guidelines are in the Theatre Ops Mgrs office}

ENTANGLEMENT

Can anyone’s hair, clothing, gloves, necktie, jewellery, costume or other materials become entangled with moving parts of the stage equipment whilst in motion?

CRUSHING

Can anyone be crushed due to:
  • Falling materials?
  • Normal or unexpected movement of any stage equipment?
  • Inability to slow or stop stage equipment?
  • Stage equipment tipping or falling over?
  • Stage equipment collapsing?
  • Coming into contact with moving parts or being thrown off stage equipment?
  • Being trapped between stage equipment?

CUT STAB OR PUNCTURE

Can anyone be cut, stabbed or punctured due to:
  • Coming into contact with sharp or flying objects?
  • Coming into contact with moving parts or moving stage equipment?

AMPUTATION

Can anyone’s body part be amputated between two parts of stage equipment, or between a part of stage equipment and a fixed point or structure?
FRICTION
Can anyone be burnt due to contact with moving parts or surfaces of the stage equipment?

STRIKE IMPACT

Can anyone be struck by moving objects due to uncontrolled or unexpected or uncontrolled movement of stage equipment, scenery, props, fly’s etc?

HYDROLICS AND COMPRESSED AIR

Can anyone come in contact with compressed air or hydrolics due to staging elements or equipment failure or misuse.

EXPLOSION

Can anyone be injured by an explosion of any substance or by theatrical type effects (e.g pyrotechnics)?

ERGONOMIC (LX, Sound and Stage Managers desks)

Can anyone be injured due to poorly designed seating, repetitive movement, poor lighting, poor body posture and the need for excessive effort?

ASPHIXIATION AND/OR SUFFOCATION

Can anyone be asphyxiated and/or suffocated due to lack of oxygen or atmospheric contamination?
EXTREME TEMPERATURE CONDITIONS
Can anyone come into contact with object at high or low temperature (globes, lanterns, smoke, dry ice, dimmers etc)?

OTHER HAZARDS

Can anyone be injured or suffer ill health from exposure to other hazards.
  • Use of orchestra pit – is there appropriate staffing?
  • Flying set items – how many pieces, is there appropriate staffing and skill level to operate?
  • Lighting effects – Strobe, Smoke etc?
