SERVICES COORDINATOR II

(Full Time 40 hours per week)

GENERAL DESCRIPTION/BACKGROUND

The National City Collaborative is a partnership of over fifty public and private organizations whose collective goal is to improve neighborhoods and assist residents in becoming self-sufficient through prevention and early intervention programs.

POSITION

Under the direct supervision of the Services Coordinator I, the Services Coordinator II will provide service coordination, assessment, case management, advocacy and linkages to seniors and adult residents with disabilities of the Summercrest Apartment Community, as well as residents in the nearby surrounding community. The Service Coordinator II will establish linkages with agencies and service providers in the community who serve seniors and people with disabilities. The Services Coordinator II will work and consult with property management staff and provide education to property management or other FRC staff regarding the needs and appropriate service delivery to this population.

RESPONSIBILITIES

¨  Provide intake, assessment and referral services for seniors and adults with disabilities;

¨  Provide ongoing case management for those individuals who need this service in order to link with and utilize existing community resources;

¨  Establish and maintain linkages with service providers in the community;

¨  Provide or coordinate educational sessions for residents about the resources available to them within the broader community;

¨  Monitor the ongoing services that are delivered to residents by community providers to assure that they are appropriate and that the resident is progressing;

¨  Develop and implement individual service plans for each resident who receives ongoing case management;

¨  Maintain accurate, timely and confidential documentation on all clients served;

¨  Organize volunteer support programs for residents;

¨  Assist residents to develop informal support networks with family, friends and other residents;

¨  Educate other staff of both the FRC and property management team about the needs and services available for seniors;

¨  Assure that all eligible residents are enrolled for the monthly Senior Food Bank distribution;

¨  Coordinate outreach as needed for the senior nutrition program, senior exercise program, and other classes provided;

¨  Develop and maintain an on site resource guide of service providers available to the residents;

¨  Serve as an advocate for residents to insure that they receiving all services for which they are eligible;

¨  Attend the monthly Collaborative Partners Meeting and the monthly FRC Staff Meeting;

¨  Work collaboratively with the quality assurance staff to assure that program outcome measures are met;

¨  Provide monthly input e.g., statistics, number of clients served, center activities/classes to the Services Coordinator I, for timely completion of written reports.

OTHER SKILLS AND QUALIFICATIONS

¨  Bilingual required (Spanish/English).

¨  A minimum of two years of experience in social service delivery with senior citizens and people with disabilities.

¨  Demonstrated working knowledge of supportive services for senior citizens and adults with disabilities available in the local community;

¨  Demonstrated ability to advocate, organize, problem-solve, and obtain results for seniors;

¨  Proven ability to work independently in an off-site location of the Family Resource Centers;

¨  Computer literacy Microsoft Office and case-management software.

¨  Ability to work flexible hours, including occasional evening or weekend work, as the position may require.

¨  Excellent oral and written communication skills.

TRANSPORTATION

A valid California Driver’s license and the daily use of a personal, reliable, insured vehicle is required.

EDUCATION

A Bachelor’s degree in the social sciences or gerontology is required. An MSW or Master’s degree in a closely related field is preferred.

CLEARANCE

Will be required to submit TB clearance, submit to fingerprinting.

WAGE INFORMATION

This is a salaried, full time, benefited position. The starting salary is $35-37K per year.
